痔围手术期不应用抗生素可行性研究

[摘要] 目的 观察痔手术前后不应用抗生素的可行性。方法 选择我院收治需行痔手术的患者90例，随机分为A、B、C三组，其中A组患者不应用抗生素，B组患者仅术前2 h内单次静脉滴注抗生素，C组患者术前、术后第1天和术后第2天静脉滴注抗生素。观察三组患者术中出血、手术时间以及术后感染情况，手术前后外周血白细胞变化情况及住院时间等。结果 三组患者手术时间、住院时间比较差异无统计学意义（P>0.05），手术前后各组外周血白细胞变化差异均无显著性（P>0.05）；三组患者术后均无感染发生。结论 痔围手术期不应用抗生素是安全可行的。

[Abstract] Objective To investigate the feasibility of no antibiotics before and after operation of hemorrhoids. Methods Ninety patients admitted to our hospital for hemorrhoids operation were randomly divided into three groups，group A patients without antibiotics，patients in group B were only 2 hours before a single intravenous infusion of antibiotics，patients in group C before，after the first day and the second day after intravenous antibiotics. The three groups were observed in patients with bleeding，operation time and postoperative infection after operation，the changes of peripheral leukocytes，hospitalization time etc. Results Three patients operation time，hospitalization time had no significant difference（P>0.05）； There was no significant difference in peripheral blood leukocyte changes before and after the operation of all groups （P>0.05），the three group patients were no infection. Conclusion No antibiotics in peri operation period of hemorrhoids is safe and feasible.

痔是肛肠科常见病、多发病，发病率约为人口总数的40%左右，占肛肠疾病构成的87.25%[1]；治疗痔的方法虽然很多，但病情较重者往往需要手术治疗。痔围手术期应用抗生素预防感染，是目前的常规治疗措施；而应用抗生素会引起许多不良反应，为避免抗生素的毒副作用，观察痔围手术期不应用抗生素的可行性，我们进行了积极的临床尝试，取得了满意效果，现报道如下。

1资料与方法

1.1 临床资料

选择2012年5月～2014年3月我院收治的90例患者，其中男42例，女48例；年龄17～76岁，平均（41.2±11.8）岁；病程5～40年，平均（33.1±6.3）年；混合痔Ⅱ度19例，Ⅲ度33例，Ⅳ度38例。随机分为A、B、C三组，三组患者在性别、年龄、病程、病情方面差异无统计学意义（P>0.05），见表1。

1.2 病例入选标准

按照《痔临床诊治指南（2006）版》[2]中相关标准确诊。排除：①合并肛门湿疹、肛瘘、肛周感染等肛门疾病者；②精神疾病……
