冠心病慢性心力衰竭患者外周血CD4+CD25+Foxp3+Treg细胞的研究

摘要：目的测定冠心病慢性心力衰竭患者的免疫功能及血浆N端脑钠肽前体（NTproBNP）水平，评价冠心病慢性心力衰竭的免疫激活状态。方法收集2013年8月—2014年2月，中国中医科学院广安门医院心内科的住院患者125例，其中慢性心力衰竭患者93例，非心力衰竭患者32例。采用化学发光免疫方法测定血浆NTproBNP水平，流式细胞技术测定外周血 CD4+CD25+Foxp3+Treg细胞比例。结果冠心病慢性心力衰竭患者血浆NTproBNP高于冠心病非心衰组，而外周血CD4+CD25+Foxp3+Treg细胞比例低于非心力衰竭组。结论冠心病慢性心力衰竭患者存在免疫激活状态，且血浆NTproBNP水平随NYHA心功能分级增加而升高。

Study of Peripheral Blood CD4+ CD25+Foxp3+Treg Cell in Patients with Coronary Heart Disease and Chronic Heart Failure

Abstract：ObjectiveTo observe the immune function and plasma Nterminal proBtype natriuretic peptide （NTproBNP） levels，and to evaluate the state of immune activation in in patients with coronary heart disease（CHD） and chronic heart failure（CHF）.MethodsFrom August 2013 to February 2014 in department of cardiology of Guanganmen hospital of traditional Chinese academy， a total of 125 inpatients， of whom 93 patients with CHF，32 patients with nonheart failure were chosen.The level of plasma NTproBNP was determined by chemiluminescence immunoassay method.The peripheral blood CD4+ CD25+Foxp3+Treg cell ratio was measured by flow cytometry.ResultsThe levels of plasma NTproBNP in patients with CHF were higher than that in patients with nonheart failure.The peripheral blood CD4+CD25+Foxp3+Treg cells in patients with CHF was lower than that in patients with nonheart failure.ConclusionThere is the presence of immune activation state in patients with CHF and the levels of plasma NTproBNP increase with increasing New York Heart Association （NYHA） functional class.

心力衰竭是各種心脏疾病发展至终末阶段的一种综合征。心力衰竭的发病机制十分复杂，近年来的研究发现，在心力衰竭患者的外周血中存在大量活化的炎症因子且存在免疫激活[1]。本课题欲通过研究外周血调节性T细胞，以阐述慢性心力衰竭过程中的免疫激活状态。

1资料与方法

1.1研究资料

1.1.1研究对象收集2013年8月—2014年2月，中国中医科学院广安门医院心内科的住院患者125例，其中慢性心力衰竭患者（CHF组）93例，男性45例，女性48例，年龄（71.09±3.97）岁;纽约心脏病学会（NYHA）Ⅱ级25例（男性13例，女性12例），年龄（66.36±7.81）岁;Ⅲ级35例（男性15例，女性20例），年龄（72.03±2.13）岁;Ⅳ级33例（男性15例，女性18例），年龄（70.76±4.12）岁。非心力衰竭患者（非CHF组）32例，男性14例，女性18例，年龄（67.90±4.43）岁，NYHA心功能为Ⅰ级。
