中西医结合治疗病毒性心肌炎快速心律失常40例临床观察

【摘要】目的观察滋阴养血清火汤联合西药治疗病毒性心肌炎快速心律失常临床疗效。方法将80例病毒性心肌炎快速心律失常患者随机分为2组。对照组40例予常规西药加盐酸胺碘酮片治疗，治疗组40例在对照组治疗基础上加用滋阴养血清火汤治疗。2组均1个月为1个疗程，1个疗程后统计临床疗效，并观察2组治疗前后中医证候评分及肿瘤坏死因子α(TNF-α)、超氧化物歧化酶(SOD)变化。结果治疗组总有效率92.50%，对照组总有效率75.00%，治疗组疗效优于对照组(P<0.05)。2组治疗后中医证候评分均降低(P<0.05)，且治疗组变化更明显(P<0.05)。2组治疗后SOD升高、TNF-α降低(P<0.05)，且治疗组优于对照组(P<0.05)。结论滋阴养血清火汤联合西药治疗病毒性心肌炎快速心律失常疗效确切，并能改善患者中医证候评分，提高SOD活性，降低TNF-α的含量。

Abstract【】ObjectiveTo investigate the clinical effects of Ziyin Yangxue Qinghuo decoction combined with Western medicine on viral myocarditis rapid arrhythmia.Methods 80 patients with viral myocarditis rapid arrhythmia were randomly divided into two groups.40 patients in control group were treated by routine Western medicine combined with amiodarone hydrochloride tablets.40 patients in treatment group were treated by Ziyin Yangxue Qinghuo decoction on the basis of the control group treatment.The course was one mouth in two groups.Clinical curative effect of two groups was evaluated after a course.The changes of traditional Chinese medicine syndrome score, tumor necrosis factor-α(TNF-α) and superoxide dismutase (SOD) before and after treatment were observed in two groups.Results The total effective rate in treatment group (92.50%) was superior to that in control group (75.00%, P<0.05).The score of traditional Chinese medicine syndrome evaluation after treatment was decreased in two groups (P<0.05), the decrease in treatment group was superior to that in control group (P<0.05).The level of SOD after treatment was increased in two groups (P<0.05), and TNF-α was decreased (P<0.05).The changes of SOD and TNF-α in treatment group were superior to those in control group (P<0.05).Conclusion Combined therapy of Ziyin Yangxue Qinghuo decoction and Western medicine on viral myocarditis rapid arrhythmia has definite curative effect, can improve traditional Chinese medicine syndrome score, increase SOD activity and decrease TNF-α content.

心律失常属中医学心悸、怔忡范畴[1]。心悸与怔忡在病因及程度上有差别，前者多因惊恐、恼怒所诱发，全身情况较好，发作时间短，病情较轻，后者则外无所惊，而自觉心悸不安，稍劳即发，全身情况较差，病情较重。2011-10—2014-04，笔者应用滋阴养血清火汤联合西药治疗病毒性心肌炎快速心律失常40例，并与常规西药加用盐酸胺碘酮片治疗40例对照观察，结果如下。

1资料与方法

1.1一般资料全部80例均为我院内二科门诊患者，随机分为2组。治疗组40例，男22例，女18例；年龄21～53岁，平均(39.3±16.5)岁；病程2～28个月，平均(14.6±5.1)个月；窦性心动过速13例，窦性心动过速伴室性心律失常(室性期前收缩)7例，窦性心动过速伴室上性心律失常14例，阵发性室上性心动过速6例。……
