网格化管理模式在回龙观社区精神障碍患者管理中的应用

目的总结网格化管理模式在管理回龙观社区精神障碍病人中的应用。方法于2014年1月—2015年12月在北京市回龙观社区对辖区内采用网格化管理模式进行实践，尤其针对精神障碍患者。结果通过网格化管理模式，开展多种形式的活动，取得了一定成绩，2014年该院各社区站对所有精神患者进行定期随访并网报，共随访患者1710人次，网报1235人次；2015年通过北京市精神卫生信息系统审核确认新发现精神疾病患者22人，迁入12人，迁出4人，死亡8人。结论网格化管理模式在管理回龙观社区精神障碍病人中应用中取得了良好的效果，值得推广。

［Abstract］Objective To summarize the application of grid management model in managing patients with mental disorder in Huilong community.Methods The area in Huilongguan community adopted the grid management model for practice from January 2014 to December 2015，especially for patients with metal disorder.Results The multi-form activities by grid management model obtained a certain achievement，1710 patients with mental disorder were regularly followed up by various communities in our hospital in 2014，and 1235 patients were reported on the internet，22 patients with mental disease were confirmed by the Beijing mental health information system in 2015，12 patients moved in，4 patients moved out，and 8 patients died.Conclusion The effect of grid management model in managing patients with mental disorder in Huilong community is good，which is worth promotion.

社区护理服务网格化管理模式，是运用网格的管理思想，针对社区医疗特点，综合运用上门服务、家庭病床、全科门诊以及社区护理门诊等多种形式，利用社区医护人员的专业互补来形成集医疗、护理、公共卫生为一体的社区卫生服务模式，在公共卫生服务体系基础上建立与街道社区联动的网格化社区护理服务体系，确保职责明确、信息共享、渠道畅通、服务有效［1-3］。目前，中国各地多省市均已开启社区护理服务网格化管理模式［4］，回龙观社区医护人员于2014年1月—2015年12月对该辖区内精神障碍患者采用网格化管理模式进行实践，取得了一定成果，现报道如下。……
