HemoCue Hb 201+ 分析仪测定血红蛋白结果的可比性研究

[摘要] 目的 评估便携式HemoCue Hb 201+分析仪急性失血后输血的指导意义。 方法 选择2013年6月～ 2015年5月间深圳市第二人民医院全麻术中因急性失血可能需要输血的患者共208例，血标本采集后，分别采用HemoCue Hb 201+分析仪及SYSMEX XE-2100型血细胞分析仪测定同一标本的Hb，所测得的数据进行相关分析，并采用Bland-Altman 法评价两种检验方法的一致性。 结果 HemoCue Hb 201+分析仪和中心实验室SYSMEX XE-2100全自动血液分析仪测得的Hb具有良好的相关性（y = 15.9297 + 0.8086x，r=0.888，P<0.001）。两种方法检测Hb差值95%的一致性限度超出临床可接受范围。 结论 HemoCue Hb 201+分析仪在急性失血后Hb的检测具有较好的准确性，并且迅捷，对术中输血具有良好的指导意义；但与标准方法的一致性尚有不足，二者不能相互取代。

[Abstract] Objective To assess the guiding significance of portable HemoCue Hb 201+ analyzer in blood transfusion after acute blood loss. Methods 208 patients with blood transfusion after acute blood loss during general anesthesia from June 2013 to May 2015 in the second people's hospital of shenzhen were selected， after blood specimen collection， the Hb of same specimen was respectively tested by HemoCue Hb 201+ analyzer and SYSMEX XE-2100 blood cell analyzer simultaneously， the taken data was relatively analyzed， the consistency between the two testing methods was evaluated by Bland-Altman method. Results The measured Hb of HemoCue Hb 201+ analyzer and SYSMEX XE-2100 automatic blood analyzer in central Laboratory had good correlation （y=15.9297+0.8086x， r=0.888， P<0.001）. The consistency limit of the 95% Hb difference tested by the two methods was beyond the clinically acceptable range. Conclusion HemoCue Hb 201+ analyzer in Hb testing after acute blood loss had good accuracy and fast， which had good guiding significance for intraoperative blood transfusion. This was inadequate compared the consistency of standard method， these two methods could not replace each other.

在外科手术过程中，选择一种简便、快速、准确的血红蛋白（Hb）测定仪器实时掌握术中血红蛋白的变化，以指导术中合理输血，是麻醉医生面临的一大难题[1-2]。在临床工作中，通常的流程是，临床工作人员采集血液标本后由相关人员送入中心实验室，中心实验室多采用全自动血细胞分析仪测定血红蛋白。这种方法虽结果准确，但所需时程长，不能作为手术过程中实时血红蛋白变化的监测[3-5]。随着科技的发展，多种床旁便携式血红蛋白分析仪已应用于许多医院的麻醉科以及ICU等，以获得血

红蛋白的实时检验数据[5]。HemoCue Hb 201+是近年来临床应用较多的一种床旁快速血红蛋白监测仪器。我们应用此仪器测定Hb，将其测定结果与中心实验室的SYSMEX XE-2100全自动血液分析仪检测结果的作对比，评价两种仪器结果的相关性和一致性，从而为HemoCue Hb 201+ 分析仪应用于临床术中输血评估提供依据。……
