牙体牙髓病的多次根管治疗与一次性根管治疗的效果对比

摘 要 目的：比较对牙体牙髓病患者采用多次根管疗法与一次性根管疗法的效果。方法：收集2014年3月—2016年8月治疗的牙体牙髓病患者50例，随机分为对照组和观察组各25例。对照组接受多次根管治疗，观察组接受一次性根管治疗，比较两组临床疗效、疼痛程度评分以及并发症发生率。结果：治疗后观察组的总有效率为96.0%（24/25），对照组为72.0%（18/25），组间差异有统计学意义（P<0.05）。在改善疼痛方面，观察组的优良率为92.0%（23/25），对照组为64.0%（16/25），组间差异有统计学意义（P<0.05）。观察组的并发症发生率为12.0%（3/25），明显低于对照组的40.0%（10/25，P<0.05）。结论：一次性根管治疗能使牙体牙髓病患者获得更佳疗效，并降低治疗后的疼痛程度和并发症发生率，有助于患者的预后，值得推广应用。

Comparison of the effects of multiple and disposable root canal therapies in the treatment of dental pulp diseases

ABSTRACT Objective： To compare the effect of multiple and disposable root canal therapies in the treatment of the patients with dental pulp diseases. Methods： Fifty cases of dental pulp disease treated in the hospital from March 2014 to August 2016 were collected and divided into a control group and an observation group with 25 cases each. The control group received multiple root canal therapy and the observation group received disposable root canal treatment. The clinical treatment effects， pain scores and rates of complications were compared. Results： After treatment， the total efficiency of the observation group was 96.0%（24/25） and that of the control group was 72.0%（18/25） and the difference between the two groups was statistically significant（P<0.05）. In the improvement of pain， the excellent rate of the observation group was 92%（23/25） and that of the control group was 64%（16/25） and the difference between the two groups was statistically significant（P<0.05）. The complication rate of the observation group was 12%（3/25）， which was significantly lower than that of the control group 40.0% （10/25， P<0.05）. Conclusion： Disposable root canal therapy can make patients with dental pulp disease get better treatment effect and reduce the degree of pain and complications after treatment， which helps to improve their prognosis and it is worthy of popularization and application.

牙體牙髓病包括龋病、牙齿硬组织磨损、根尖周病、牙齿感觉过敏症等，由于患牙出现的剧烈疼痛，对患者的生活、工作甚至睡眠造成严重影响[1]。有研究报道牙体牙髓病常由专性厌氧菌、兼性厌氧菌等感染引发，部分患者可由物理刺激、药物刺激、创伤等因素而发病[2]。根管治疗是牙体牙髓病、根尖周病的主要治疗方法，可清除已坏死的牙体和牙髓，进行密实填充，从而有效消灭感染路径，并促进组织愈合[3]。但是，传统的多次根管疗法治疗周期较长，患者需频繁就诊，因此治疗依从性不高[4]。本研究比较多次根管疗法和一次性根管疗法治疗牙体牙髓病的疗效，现报道如下。
