基于鼻中隔后动脉的改良Millard术式在双侧完全性唇裂中的应用研究

[摘要] 目的 探讨基于鼻中隔后动脉的改良Millard术式治疗双侧完全性唇裂畸形的临床疗效。方法 方便选择2015年1月—2016年12月蚌埠医学院第一附属医院口腔科收治的双侧完全性唇裂患儿13例，基于鼻中隔后动脉为解剖学基础的改良Millard法，根据双侧完全性唇裂及鼻唇畸形的严重程度来决定是否对术式进行改良，依据个体差异设计不同的操作细节进行修复。结果 13例患儿均为一期愈合，随访3、6个月和（或）12个月，唇、鼻部外形随着颌面部的生长发育而得到改善。结论 应用改良Millard方法修复双侧完全性唇裂，是一种综合修复的良好方法。

Application of Modified Millard Procedure in Bilateral Complete Cleft Lip Based on Post Nasal Septal Arteries

[Abstract] Objective This paper tries to investigate the clinical effect of modified Millard surgical treatment of bilateral complete cleft lip deformity based on posterior nasal septum artery. Methods 13 cases of children with bilateral complete cleft lip were convenient selected from January 2015 and December 2016 in this hospital. Based on the modified Millard method， based on the post-septum artery， and the severity of the bilaterally consistent lip and nose deformities to determine whether or not to modify the procedure， to design the procedure to repair the procedure in a different way. Results All were in primary healing， lips and nose shape with the maxillofacial growth were improved， the long-term effect was considerable when follow-up of patients for three months and six months and （or） twelve months. Conclusion Improved Millard method applied to repair bilateral complete cleft lip， is a good method of maintaining dynamic effect.

双侧完全性唇裂是唇裂畸形中最为严重的类型，呈现为较多的组织缺损和明显的组织移位，临床上主要采用的整复方法有前唇加高的矩形瓣法（rectangular flap method），原长直线法（straight line repair）和旋转推进法（Millard ‘s method）等[1]，就其术后效果来说，多不尽如意，双侧唇裂术后继发畸形长包括，上唇过紧、人中过宽、鼻小柱过短等[2-3]。一期手术虽恢复了静态的外形，但动态的美很不协调， 如何将其各种方法的潜在优势发挥起来，使其获得满意的效果是专家、学者们的不懈追求。作者近年在双侧完全性唇裂的整复工作中非常青睐“旋转推进法”，经对术式的部分改良、前唇组织长度和宽度取舍及鼻部游离等方面的不断尝试，总结一些个体化治疗的经验以飨读者，现报道如下。……
