肺曲霉菌病误诊为肺癌的临床分析

【摘要】目的： 分析肺曲霉菌病的临床表现并结合影像学特点。 方法：回顾性分析对我院自2014年3月～2017年12月确诊的肺曲霉菌病30例的诊治，并进行进一步分析。 结果：肺曲霉菌病的症状主要表现为咳嗽19例；咯白粘痰13例；发热15例；胸闷气短8例；咳血痰7例，干咳4例。结合影像学改变主要表现为结节病灶6例，空洞样病灶3例，块样球型病灶6例；我们结合CT增强扫描，明显强化者7例。确诊方法：痰培养。 结论：肺曲霉菌病的临床症状不典型，影像学显示欠佳。依靠病理组织活检有重要的诊断价值。

Clinical treatment and observation of systemic lupus erythematosus

Abstract Objective：to analyze the clinical manifestations and imaging features of pulmonary aspergillosis.Methods： The diagnosis and treatment of 30 cases of pulmonary aspergillosis diagnosed in our hospital from March 2014 to December 2017 were retrospectively analyzed and analyzed further.Results：The symptoms of pulmonary aspergillosis were mainly 19 cases of cough， 13 cases of phlegm and phlegm， 15 cases of fever， 8 cases of chest tightness and shortness of breath， 7 cases of cough and sputum， and 4 cases of dry cough.Combined with imaging changes， the main manifestations were nodular lesions in 6 cases， cavity like lesions in 3 cases， and block like nodular lesions in 6 cases.We combined CT enhanced scan and 7 cases with obvious intensification.The method of diagnosis： sputum culture. Conclusion： The clinical symptoms of pulmonary aspergillosis are not typical， and imaging findings are poor.It is important to rely on pathological biopsy.

近年來，随着免疫缺陷患者的增加，皮质类固醇，免疫抑制剂的应用以及抗肿瘤药物的应用，深部真菌的发病率逐年增加，曲霉菌是仅次于念珠菌的常见病原菌。肺曲菌病为院内感染所致的深部真菌感染的一个重要原因[1]。对本院自2014年3月至2017年12月收住的30例肺曲霉菌感染患者，分析其临床特点，报道如下。

1材料和方法

11一般资料我院自2014年3月～2017年12月确诊的肺曲霉菌感染患者30例，其中男21例，女9例，年龄23～61岁。确诊前病程5～18个月。

12确诊方法3例患者均经活及痰培养组织病理诊断，其中肺组织穿刺13例， 纤维支气管镜下活检10例。其中纤微支气管镜抽吸病灶部位分泌物或以灌洗液做霉菌培养或涂片16例找到病原菌。

13临床特征①症状： 咳嗽19例；咯白粘痰13例；发热15例；胸闷气短8例；咯血痰7例，干咳4例。②体征：双肺局部干罗音16例，局部湿罗音6例，浅表淋巴结肿大者8例。……
