单根锉Wave One镍钛器械用于治疗弯曲根管的临床效果

[摘要] 目的 觀察单根锉Wave One镍钛器械用于治疗弯曲根管的临床效果。方法 采用临床资料查证法及充填后疗效观察法。方便选取2014年9月—2016年9月以来该院口腔科诊治的98例磨牙急、慢性根尖周炎和牙髓炎患者，所有磨牙牙根都有一定的弯曲度。按照预备根管所采用的器械不同，等分为对照组和观察组，分别采用传统不锈钢K锉和单根锉Wave One镍钛器械，都采用侧压方式进行根管充填，观察两组用于治疗弯曲根管的临床效果。记录根管预备过程中台阶形成，根尖拉开和治疗时间，比较根管成形和充填效果，随访充填后疼痛情况。结果 对照组优良率85.71%（42/49）明显低于观察组优良率97.96%（48/49），两组数据差异有统计学意义（χ2=11.334 9，P=0.049 6 <0.05），观察组行根管治疗术的时间比对照组明显缩短，大大提升临床效率。 结论 单根锉Wave One镍钛器械用于治疗弯曲根管的临床效果确切，比之传统不锈钢K锉有着更高的临床疗效优良率，且大幅减少治疗时间，值得大力推广。

Clinical Effect of One File Endo Wave One Nickel Titanium Instrument in Treatment of Curved Root Canal

[Abstract] Objective To observe the clinical effect of one file endo Wave One nickel titanium instrument in treatment of curved root canal. Methods 98 cases of patients with molar acute and chronic periapical periodontitis and pulpitis admitted and treated in our hospital from September 2014 to September 2016 were convenient selected， and all the molar root had a certain curved degree， and the patients were divided into 49 cases in each according to different device， respectively treated with traditional stainless steel k files and one file endo Wave One nickel titanium instrument， both groups used the lateral pressure method for root canal filling， and the clinical effect of the two groups was observed， and the step formation， apical zip and treatment time in the root canal preparatory process were recorded， and the step formation and filling effect were compared， and the pains score after filling was followed up. Results The excellent and good rate in the control group was obviously lower than that in the observation group， [85.71%（42/49） vs 97.96%（48/49）]， and the difference was statistically significant （χ2=11.334 9， P=0.049 6 <0.05）， and the root canal treatment time in the observation group was obviously shortened compared with that in the control group， and it could greatly improve the clinical effect. Conclusion The clinical effect of one file endo Wave One nickel titanium instrument in treatment of curved root canal is definite， and the clinical curative excellent and good rate is higher than that of the traditional stainless steel k files， and it can greatly reduce the treatment time， and it is worth great promotion.

急、慢性根尖周炎和牙髓炎的治疗，惯常采用根管治疗术（root canal therapy，RCT），而弯曲根管是根管治疗术中的难点，是根管治疗术中根管通畅度及根管解剖形态稳定的主要限制因素，使用恰当的器械辅助治疗，对确保根管治疗的成功，具有切实有效的效果[1]。传统不锈钢K锉和单根锉Wave One镍钛器械作为两种常见方法，实施不同的根管预备方式，不锈钢K锉使用逐步后退法，因为器械硬度高，预备时需要预弯器械，操作时注意握住手指，控制器械方向，扩锉的部位尽可能接近根尖弯曲点，尽量避免并发症的出现。Waveone镍钛锉采用冠向下方法预备，器械柔韧性比较好，不易折断和台阶形成，更适合弯曲根管的预备。……
