心脏彩色多普勒超声诊断高血压性心脏病的应用价值

[摘要] 目的 分析高血压性心脏病应用心脏彩色多普勒超声的诊断效果。方法 方便选择该院2014年1月—2016年4月收治的103例高血压性心脏病患者，所有患者均采用心脏彩色多普勒超声检查，综合患者临床表现、病史及实验室检查结果分析心脏彩色多普勒超声检查的准确性及图像特征。 结果 103例患者心脏彩色多普勒超声诊断阳性率为98.06%（101/103），有2例漏诊;103例患者图像测量指标显示，平均左房内径（3.12±0.27）mm、左房内径/主动脉内径（1.14±0.09）、射血分数（61.28±7.33）%、E/A值（1.51±0.10），超声图像提示会出现不同程度的左房增大、左室肥厚、主动脉扩张、心律失常等。 结论 心脏彩色多普勒超声诊断可较为直观了解高血压性心脏病患者的血流信号情况，阳性检出率高，有效提示心血管病变情况，是高血压性心脏病的理想影像学诊断方法。

The Value of Cardiac Color Doppler Ultrasound in Ddiagnosis of Hypertensive Heart Disease

[Abstract] Objective To analyze the diagnostic effect of cardiac color Doppler ultrasound in hypertensive heart disease. Methods 103 patients with hypertensive heart disease who were admitted to the hospital from January 2014 to April 2016 were convenient selected. All patients were evaluated by cardiac color Doppler ultrasonography. The clinical manifestations， medical histories， and laboratory examination results of the patients were analyzed. Doppler ultrasound accuracy and image characteristics. Results The positive rate of color Doppler sonography in 103 patients was 98.06%（101/103）. There were 2 missed diagnoses. The image measurements of 103 patients showed that the average left atrial diameter was （3.12±0.27）mm and the left atrium was Aortic diameter （1.14 ± 0.09）， ejection fraction （61.28 ± 7.33）%， E/A value （1.51±0.10）， ultrasound images suggested varying degrees of left atrial enlargement， left ventricular hypertrophy， aortic dilatation ， arrhythmia， etc. Conclusion Cardiac color Doppler ultrasound diagnosis can be used to intuitively understand the blood flow status of patients with hypertensive heart disease. The positive detection rate is high， which effectively indicates the status of cardiovascular diseases. It is an ideal imaging diagnostic method for hypertensive heart disease.

高血压性心脏病是心血管系统的常见病，是由于循环体动脉在长期的高压状态下导致心脏负荷增加而引起的病变。高血压性心脏病初期无明显特异性表现，胸闷、气短、高血压等临床症状也往往容易被忽视[1]。然而，高血压性心脏病患者由于心脏负荷过重，会出现有左心室肥厚，可引起患者出现心功能衰退，可会激发出现ST-T段心电图改变，临床临床症状加剧，严重威胁的患者生命健康。高血压性心脏病发病率高，患者多为中老年群体，患者心功能和免疫调节能力减弱，进一步加剧了病情恶化的风险，因此早诊断早治疗对于提高临床疗效、改善预后具有重要作用[2]。……
