静脉应用蔗糖铁治疗慢性肾衰竭患者肾性贫血的临床观察

【摘要】 目的：探讨蔗糖铁在慢性肾衰竭合并肾性贫血患者中的应用疗效。方法：选取笔者所在医院2017年4月-2018年6月收治的92例慢性肾衰竭合并肾性贫血患者作为研究对象，分为观察组（蔗糖铁+促红细胞生成素）和对照组（促红细胞生成素），各46例，观察两组患者的治疗前后血红蛋白（Hb）、转铁蛋白饱和度（TSAT）、血细胞比容（Hct）及血清铁蛋白（SF）的变化。结果：与治疗前相比，两组患者治疗后的Hb、TSAT、Hct及SF等指标水平均提高（P<0.05）;与对照组相比，观察组患者相对更高（P<0.05）。观察组总有效率95.62%，高于对照组的73.91%，差异有统计学意义（P<0.05）。结论：静脉应用蔗糖铁是治疗慢性肾衰竭合并肾性贫血的良好选择，其疗效和安全性方面有着显著的优势。

[Abstract] Objective： To investigate the efficacy of sucrose iron in patients with chronic renal failure complicated with renal anemia. Method： From April 2017 to June 2018， 92 patients with chronic renal failure complicated with renal anemia were divided into observation group （n=46） and control group （n=46）. The changes of hemoglobin （Hb）， transferrin saturation （TSAT）， specific volume （Hct） and serum ferritin （SF） were observed before and after treatment. Result： Compared with before treatment， the levels of Hb， TSAT， Hct and SF in the two groups were higher than those in the control group （P<0.05）， and those in the observation group were higher than those in the control group （P<0.05）. The total effective rate in the observation group was 95.62%， which was significantly higher than 73.91% in the control group （P<0.05）.Conclusion： Intravenous application of sucrose iron is a good choice for the treatment of chronic renal failure complicated with renal anemia， and its efficacy and safety have significant advantages.

慢性腎衰竭为多种慢性肾脏疾病引发的病症，由于患者的肾脏功能受到损害，引起代谢紊乱，发生代谢废物潴留等情况，并会损害其血液、心血管及神经等多个系统的功能，产生相关并发症[1-2]。由于慢性肾衰竭患者肾脏功能受损，其肾小球滤过率低下，促红细胞生成素的生成减少，进而引发肾性贫血。与此同时，受到疾病的影响，患者的骨髓微环境受到破坏，并存在着铁元素缺失、出血、失血等情况，导致肾性贫血症状的加剧。肾性贫血作为慢性肾衰竭的主要并发症之一，不但严重影响患者的生活质量，同时也是患者心脏事件及死亡的危险因素。因此慢性肾衰竭的贫血管理是慢性肾衰一体化治疗的重要组成部分。需要关注患者的肾性贫血症状，了解其发生原因和影响因素，采取有效的治疗措施[3-4]。本研究探讨了92例慢性肾衰竭合并肾性贫血患者的临床治疗方法，观察静脉应用蔗糖铁的疗效，现报告如下。……
