Phaco或ECCE联合小梁切除术治疗白内障合并青光眼的对照研究

【摘 要】目的：探析Phaco与ECCE联合小梁切除术治疗白内障合并青光眼的临床疗效。方法：将我院收治的80例白内障合并青光眼患者设为研究对象，取数字表随机原则分2组，分组视力、视野、散光分布无差异，即对照组40例行ECCE联合小梁切除术，分析组40例行Phaco联合小梁切除术治疗，总结两组患者临床治疗情况。结果：①手术前两组患者散光度、眼压水平差异不明显（P>0.05），经手术治疗后分析组改善效果明显优于对照组，差异具有统计学意义（P<0.05）；②从两组视力恢复情况可见，分析组视力恢复率明显高于对照组，视力下降率明显低于对照组，差异均具有统计学意义（P<0.05）；③经术后统计学显示分析组角膜水肿、前房积血、前房渗出等并发症发生率明显低于对照组，差异具有统计学意义（P<0.05）。结论：与ECCE联合小梁切除术治疗白内障合并青光眼相比，Phaco联合小梁切除术治疗疗效更优，具有重要推广应用价值。

A Comparative study of Phaco or ECCE combined with trabeculectomy in the treatment of Cataract complicated with Glaucoma

Abstract：Objective To explore the clinical efficacy of Phaco and ECCE combined with trabeculectomy for cataract complicated with glaucoma. Methods： 80 cataract patients with glaucoma in our hospital were divided into two groups randomly. There was no difference in visual acuity， visual field and astigmatism distribution between the two groups. In the control group， 40 cases were treated with ECCE combined with trabeculectomy， and 40 cases in the control group were treated with trabeculectomy. 40 cases in the analysis group were treated with Phaco combined with trabeculectomy， and the clinical treatment of the two groups were summarized. Results： 1 there was no significant difference in astigmatism and IOP between the two groups before operation （P > 0.05）. The results were significantly better than those of the control group （P<0.05）. （2） the recovery rate of visual acuity in the analysis group was significantly higher than that in the control group， and the decline rate was significantly lower than that in the control group （P<0.05）. 3The incidence of corneal edema， hyphema and hyphema in the analysis group was significantly lower than that in the control group （P<0.05）. Conclusion： compared with ECCE combined trabeculectomy with trabeculectomy， Phaco combined trabeculectomy has a better therapeutic effect and has an important popularization in the treatment of cataract and glaucoma. Application value.

随着显微与微创技术研究不断深入，超声乳化（Phaco）与白内障囊外摘除术（ECCE）结合小梁切除术应用于白内障合并青光眼的治疗中愈发广泛，但就两种术式联用方案的治疗效果差异仍需进一步研究阐述。鉴于此，本次研究将我院收治的80例白内障合并急、慢性青光眼患者设为研究对象，探析Phaco与ECCE联合小梁切除术治疗白内障合并青光眼的临床疗效，旨在为临床术式选择提供参考，总结如下：

1 资料与方法

1.1 一般资料 本次研究将2011年5月——2017年7月间我院收治的80例白内障合并急、慢性青光眼患者设为研究对象；纳入标准：经WHO诊断标准确诊，青光眼的诊断由眼内压（Iop）、杯盘比（C/D）、OCT与视野，图形视觉诱发电位完成检查确诊；排除标准：排除存在心脑血管疾病者、神经疾病者、治疗依从性较差者、意识不清晰者、不愿接受本次对比研究者；

现遵循数字表随机法原则分为对照组与分析组各40例，对照组中男26例，女14例，年龄40～75岁，平均年龄（66.9±7.9）岁，平均眼压（34.9±10.3）mmHg，平均散光度（0.90±0.12）D；分析……
