温针结合中药穴位敷贴治疗寒湿型膝关节炎疗效观察

摘 要 目的：观察温针结合中药穴位敷贴对寒湿型膝关节炎的临床疗效。方法：收集2017年1月—2018年6月收治的寒湿型膝关节炎患者70例，随机分为治疗组和对照组各35例。治疗组予以温针结合中药穴位敷贴治疗，对照组予以常规取穴针灸治疗。两组均为隔天治疗1次，10 d为1疗程，治疗2个疗程，采用膝关节功能评定量表（HSS）分别评估两组疗效。结果：治疗组第1和第2疗程后总有效率分别为71.43%和85.71%，均高于对照组的48.57%和68.57%（P<0.05）。结论：温针结合中药穴位敷贴治疗寒湿型膝关节炎疗效较常规取穴针灸法更为显著，值得应用。

Therapeutic effect of warm acupuncture treatment combined with traditional Chinese medicine acupoint application on cold-dampness knee arthritis

ABSTRACT Objective： To observe the clinical efficacy of warm acupuncture treatment combined with acupoint application of traditional Chinese medicine on the treatment of cold-dampness knee arthritis. Methods： Seventy patients with cold-dampness knee arthritis admitted from January 2017 to June 2018 were collected and randomly divided into a treatment group and a control group with 35 cases each. The treatment group received warm acupuncture combined with traditional Chinese medicine acupoint application， whereas the control group received conventional acupuncture treatment， both given once every other day， 10 days as a course and continuous for 2 courses. The therapeutic effects of two groups were evaluated using the knee joint function assessment scale（hospital special surgery， HSS） and compared. Results： The total effective rates of the first and second course of the treatment group were 71.43% and 85.71%， respectively， higher than those of the control group（48.57% and 68.57%， P<0.05）. Conclusion： Therapeutic effect of warm acupuncture treatment combined with traditional Chinese medicine acupoint application on cold-dampness knee arthritis is more remarkable than that of conventional acupuncture treatment， which is worthy of application.

膝关节骨性关节炎是一种由于膝关节慢性劳损或局部炎症引起膝关节软骨变性及活动障碍的慢性退行性骨关节疾病。我国50岁以上的人口中，膝关节炎发病率为5%左右，60岁以上男性为15%，女性为25%，是老年人的常见多发病。其发病原因与年龄、肥胖、炎症、创伤和遗传等因素有关[1]。自2017年1月—2018年6月，作者采用温针结合中药穴位敷贴治疗寒湿型膝关节炎，现报告如下。

1 资料与方法

1.1 一般资料

收集2017年1月—2018年6月就诊于上海市嘉定区外冈镇社区卫生服务中心中医门诊的寒湿型膝关节炎患者70例，均符合《上海市中医病证诊疗常规（第2版）》[2]中有关寒湿型膝关节炎的诊断标准：（1）初起多见膝关节隐痛，遇寒加重，得热痛减；俯仰、屈伸、转侧不利，气候变化加重，舌苔薄白，脉弦紧。（2）起病隐袭，发病缓慢，多见于中老年人。（3）局部关节可轻度肿胀，活动时关节常有喀喇声，严重者可见肌肉萎缩，关节畸形。……
