妇科健康体检者液基薄层细胞学检测的应用与结果分析

【摘 要】目的：分析液基薄层细胞学检测（TCT）在妇科健康体检者筛查宫颈疾病中的应用意义。方法：选取本院2016年6月至2017年10月参与妇科健康体检的6254例女性进行TCT检查，并对TCT检查疑似为上皮细胞异常改变者进行阴道镜活检，计算TCT检查宫颈病变与阴道镜活检的病理符合率，分析TCT检查在妇科健康体检中的临床应用意义。结果：在6254例中，TCT筛查上皮细胞异常有142例，阳性检出率为2.3%。结论：妇科健康体检中进行TCT检查，早期筛查宫颈病变，并尽早指导临床治疗，可有效避免其恶变的发生。因此筛选宫颈癌前病变及宫颈癌，对癌前病变、宫颈癌的早期诊断和正确治疗有着重要的临床意义。

Application and results analysis of liquid-based thin layer cytology in gynecological health checkup

[Abstract] Objective：To analyze the application of liquid-based thin-layer cytology （TCT） in the screening of cervical diseases in gynecological health checkups. Methods： In our hospital from June 2016 to October 2017， 6254 women who participated in the gynaecological health examination underwent TCT examination， and colposcopy biopsy was performed on patients with suspected epithelial cell abnormalities in TCT examination. The TCT examination of cervical lesions and colposcopy biopsy was performed. Pathological coincidence rate， analysis of the clinical application significance of TCT examination in gynecological health examination. Results： Among the 6254 cases， TCT screened 142 cases of abnormal epithelial cells， and the positive detection rate was 2.3%. Conclusion： TCT examination in gynecological health examination， early screening of cervical lesions， and early guidance of clinical treatment can effectively prevent the occurrence of malignant transformation. Therefore， screening cervical precancerous lesions and cervical cancer has important clinical significance for early diagnosis and correct treatment of precancerous lesions and cervical cancer.

宮颈癌筛查是妇科疾病筛查的主要内容之一。宫颈癌具有高发病率，中国的发病率和死亡人数约占全球患者总数的1/3，对中国女性造成严重的生命威胁;根据相关研究报道，宫颈鳞状上皮病变可能发生在宫颈癌的早期阶段。如果在此阶段给予及时干预，大多数鳞状上皮病变将逆转正常并消除癌性隐患。因此，加强妇科疾病普查，并采取有效的检查方法，改善CIN和早期癌症筛查，以保护女性的健康。宫颈癌的发病率仅次于乳腺癌，位居女性恶性肿瘤疾病的第二位。有数据统计[1]，全球每年新增的宫颈癌病例多达46.6万人，而我国宫颈癌患者约有13万人左右。该疾病对女性健康的威胁较大，必须尽早予以诊断和治疗。另外，宫颈癌的进展中存在一个长时间的可逆性瘤前病变阶段，如能早期及时诊断、积极准确治疗，可使患者的治愈率提升至90%以上。……
