皮肤磨削术及其应用

[摘要]皮肤磨削术是在皮肤病的治疗及美容治疗中适应证较为广泛的一种治疗手段，是医学美容换肤技术在临床上最为常用的一种方法，磨削术常规是使用一些器械或设备，对表皮和真皮浅层进行可控制的机械性磨削、以完成治疗及美容的一种手术。不同类型的患者的治疗方法选择是有所区别的，医生应对不同患者选取不同的治疗方法及设备。本文通过综述结合临床病例治疗经验对皮肤磨削术的方法加以系统介绍，让大家重新认识皮肤磨削术这一古老而传统的治疗方法，对临床应用具有重要意义。

Abstract： Dermabrasion was a very effective method commonly used in the treatment of dermatological diseases. It was the most commonly used method in medical cosmetic skin rejuvenation. It refers to an operation that uses instruments or equipment to perform controlled mechanical abrasion of the epidermis and superficial dermis to achieve disease treatment or cosmetic effect. The treatment options for different types of patients are different. Doctors should choose different treatment methods and equipment for different patients. This article systematically introduces the method of dermabrasion through literature review and clinical cases， so that everyone could re-understand this ancient and traditional treatment， which is very important for clinical application.

磨削术通常是使用一种装有砂石磨头或金钢磨头的快速旋转的手持器械，对表皮和真皮浅层进行可控制的机械性磨削，以完成治疗及美容的一种手术。磨削后创面愈合过程中真皮的胶原纤维和弹性纤维重新排布，残存的皮肤附属器（毛囊、皮脂腺、汗腺）会迅速形成新的表皮，创面几乎不留有瘢痕。

现代磨皮换肤术始于德国皮肤病学家克罗梅尔（Kromayer），1930年在他的专著Cosmetic Treatment of Skin Complaints中系统描述了皮肤磨削的方法，使用动力驱动设备进行磨皮，主要用于治疗痤疮瘢痕，他研究了磨至不同深度对皮肤的影响，并且证实：如果磨皮不穿透网状真皮层，就不会产生瘢痕[1]。
