根除幽门螺杆菌治疗与肠易激综合征关系的Meta分析

【摘要】目的：探讨幽门螺杆菌（Helicobacter pylori， Hp）感染及其根除治疗与肠易激综合征（irritable bowel syndrome， IBS）的关系。方法：在Pubmed、EMBASE、Cochrance Library、中国知网、万方等数据库检索1995年至今公开发表的有关 Hp 与 IBS 的中外文文献，并对文献进行筛选，应用RevMan 5.30軟件进行 meta 分析。结果：本研究共纳入文献23篇， Meta 分析结果显示，IBS 组 Hp 感染率高于正常对照组（OR=1.67，95%CI：1.23～2.28， P =0.001），根除 Hp 感染对 IBS 患者症状改善有效率大于对照组（OR=2.08，95%CI：1.45～2.97， P<0.01）。结论： IS 患者 Hp 感染的可能性增加，根除 Hp 治疗可改善 IBS 患者症状，提示 Hp 感染可能是 IBS 发生的危险因素。

【Abstract】Objective：Toinvestigatetherelationshipbetweenhelicobacter pylori（Hp）infectionanditseradicationtreat- ment and irritable bowelsyndrome（IBS）.Methods：Searched Pubmed，EMBASE，Cochrance Library，CNKI，Wanfang and oth- er databases in Chinese and foreign literature related to Hp and IBS published from 1995 to the present，and screened the litera- ture，and applied RevMan 5.30 software for meta analysis.Results：A total of 23 documents were included in this study.The results of Meta analysis showed that the Hp infection rate in the IBS group was higher than that in the normal control group（OR=1.67，95% CI：1.23～2.28，P=0.001），and the effective rate of eradicating Hp infection in improving the symptoms of IBS patients was greater than that in the control group（OR=2.08，95% CI：1.45～2.97，P<0.01）.Conclusion：IBS patients have an increased possibility of Hp infection，and eradication of Hp treatment can improve symptoms in patients with IBS，suggesting that Hp infection may be a risk factor for THE occurrence ofIBS.

肠易激综合征（Irritable Bowel Syndrome，IBS）是国内外发病率较高的胃肠道功能性疾病，其病因及发病机制尚不清楚，以往的研究表明[1]，脑- 肠相互作用异常、肠道菌群改变、慢性低度粘膜炎症和心理障碍可能参与IBS 的病理生理学。

幽门螺杆菌（Helicobacterpylori，Hp）感染被认为是慢性胃炎、消化性溃疡、胃癌和胃粘膜相关淋巴组织淋巴瘤的主要病因。此外， Hp 可能通过触发全身炎症反应在非胃肠道疾病中发挥作用[2]。然而， Hp 感染与 IBS 之间的联系是有争议的。

1资料与方法

1.1 检索策略

计算机检索外文数据库Pubmed、EMBASE、CochranceLibrary、Web ofScience和中文数据库中国知网、万方数据库，英文检索词 Helicobacter pylori，H.pylori，Hp;Irritable BowelSyndrome，IBS。中文检索词为幽门螺杆菌，幽门螺旋杆菌;肠易激综合征。检索时间为1995年1月 1日至2020年8月31日。

纳入标准：研究设计为随机对照实验、病例对照研究或横断面研究;发表时间为1995年1月 1日至2020年 8月31日，发表语言为中文或英文;对IBS有明确的诊断依据; Hp 至少通过……
