精细化三级管理在医院感染管理中的应用

【摘要】目的：探讨精细化三级管理对住院患者院感率的影响。方法：以2017年住院患者为对照组，实施常规三级院感管理;以2018年及2019年住院患者为观察组实施精细化三级院感管理，比较两组患者的院感率。结果：观察组手卫生依从性由85.77%上升到95.28%、高频接触物体表面消毒执行率从86.58%上升到98.06%，住院患者院感率（1.15%）比实施前（2.09%）明显下降（P<0.01）。结论：在医院中实施精细化三级院感管理，可以有效降低住院患者的院感率。

[Abstract] Objective： Application of refined three level management in hospital infection management. Methods： The inpatients in 2017 were taken as the control group， and the conventional three-level hospital sense management was implemented; the inpatients in 2018 and 2019 were taken as the observation group， and the refined three-level hospital sense management was implemented， and the hospital sense rate of the two groups was compared. Result： In the observation group， the hand hygiene compliance increased from 85.77% to 95.28%， and the implementation rate of surface disinfection of high-frequency contact objects increased from 86.58% to 98.06%. The hospital infection rate of inpatients （1.15%） was significantly lower than that before （2.09%）（P<0.01）. Conclusion： The implementation of refined three-level hospital sensemanagement in the hospital can effectively reduce the hospital sense rate of inpatients.

医院三级感染管理架构是指由医院感染管理委员会、医院感染管理科、临床科室感控管理小组三个层级组成的医院感染质量控制网络，是目前大部分医院采用的管理模式。但采用同样的管理模式，在实施中却出现管理质量相距甚远的结果，其原因是以前采用的粗放式管理所导致[1]。精细化三级院感管理是指以防控技术为基础，以防控措施为特征，充分运用各种资源（包括医疗、护理、药剂、微生物室、后勤部门等）强化协作，提高院感管理质量最终达到降低院内感染率、保障患者及医务人员安全的目的。本文以2018年及2019年期间住院患者为研究对象，研究探讨精细化三级院感管理对住院患者医院感染率的影响。

1 资料与方法

1.1一般资料以 2017年住院患者作为对照组，选取2018年1月至2019年 12月期间的住院患者作为研究对象，实施精细化三级医院感染管理，观察住院患者院感率，与2017年住院患者的院感率进行比较。……
