浅析儿科支原体肺炎治疗中应用阿奇霉素与孟鲁司特钠的作用

摘要：目的：分析儿科支原体肺炎治疗中应用阿奇霉素与孟鲁司特钠的临床作用。方法：选取2020年1月～2021年7月于本院儿科就诊支原体肺炎患儿52例行对比性治疗研究，随机分组为对比组（n=27）、试验组（n=25）后，分别实施阿奇霉素单药治疗，阿奇霉素+孟鲁司特钠联合治疗。以患儿症状缓解时间、住院时间、血清炎症指标为标准，评价两组临床疗效差异。结果：试验组治疗期间退热、咳嗽、憋喘、肺部啰音缓解时间，住院时间及治疗后血清CRP低于对比组，差异显著（P<0.05）。结论：儿科支原体肺炎治疗中采用阿奇霉素、孟鲁司特钠联合治疗方案，可在快速纠正患儿临床症状同时，促进肺通气功能恢复，且可辅助抗生素治疗纠正呼吸系统感染性炎症进展，效果显著。

[Abstract] Objective： to analyze the clinical effect of azithromycin and montelukast sodium in the treatment of pediatric mycoplasma pneumonia. Methods： 52 children with mycoplasma pneumonia treated in our hospital from January 2020 to July 2021 were randomly divided into control group （n = 27） and experimental group After （n = 25）， azithromycin single drug treatment and azithromycin + montelukast sodium combined treatment were carried out respectively. The clinical efficacy differences between the two groups were evaluated according to the symptom relief time， hospital stay and serum inflammatory indexes. Results： the relief time of fever， cough， wheezing， pulmonary rales， hospital stay and serum CRP in the experimental group were lower than those in the control group Conclusion： the combination of azithromycin and montelukast sodium in the treatment of pediatric mycoplasma pneumonia can quickly correct the clinical symptoms of children， promote the recovery of pulmonary ventilation function， and assist antibiotic treatment to correct the progress of infectious inflammation of respiratory system.

支原体肺炎是儿科常见呼吸道感染疾病类型，支原体感染为根本发病机制，受疾病感染源特殊性影响，故又称为原发性非典型肺炎[1]，相较求他类型病原物感染性肺炎，支原体感染较难清除[2]，且可随感染进展、病程迁延，存在较高全身性感染风险，增加心肺功能、肝脏功能损伤风险，影响患儿临床预后及健康质量维持[3]。阿奇霉素是支原体肺炎首选抗生素治疗类型，对支原体具有显著抑制、杀灭效果，但随临床应用逐步深入后发现，阿奇霉素治疗虽可积极发挥抗感染效果[4]，但对患儿呼吸道感染症状缓解效果有限，或可由呼吸系统症状持续时间延长，延长阿奇霉素单药治疗时间增加药物不良反应风险，故在阿奇霉素基础上实施联合治疗促进患儿呼吸系统症状缓解为当前主要治疗研究方向[5]。……
