Construction and case analysis of information-assisted medication therapy management （MTM） in the pharmaceutical services of type 2 diabetes patients

[Abstract] Medication therapy management has been implemented in the United States for more than ten years， saving a lot of medical expenses， and has been recognized by the US government. In recent years， as patients′ demand for high-quality medical services has increased， my country′s attention to medication therapy management （MTM） has gradually increased. Traditional medication therapy management （MTM） is mainly based on face-to-face pharmaceutical services. This research combines information technology with the traditional face-to-face pharmacy service model to construct a new type of medication therapy management work model and conduct case analysis. The use of information-based auxiliary methods to manage medication therapy for patients with type 2 diabetes can lower the patient′s blood sugar， improve the patient′s medication compliance， and improve the level of hospital pharmacy services.

近40年来，糖尿病的患病率呈现急剧增加的趋势，糖尿病已经从少见病变为多发病和常见病[1]。我国超过10%的成年人患有糖尿病，而接受治疗的患者占49.0%，能有效控制血糖的患者仅占49.4%[2]。血糖长期控制不佳会导致糖尿病性肾病、糖尿病性眼病等并发症，严重影响患者的生存质量[3]。药物治疗管理（Medication therapy management，MTM）是由具有药学专业技术资格的药师为患者提供用药教育及咨询指导等专业的药学服务。MTM可以提高患者的用药依从性，防止用药错误，提升患者的自我管理能力[4-5]。MTM已在美国实行了十余年的时间，节约了大量的医疗费用，并获得了美国政府的认可[6]。近年来，随着患者对高质量医疗服务需求的增加，我国对药物治疗管理的关注度也在逐渐提升[7]。MTM药学服务模式主要以面对面服务模式为主，笔者将信息化手段同传统的面对面药学服务模式相结合，对2型糖尿病患者的药物治疗管理经验进行总结，并进行案例分析。

1 构建信息化辅助的2型糖尿病患者MTM工作模式

1.1 药学服务对象及信息化辅助工具

本研究的药学服务对象为18～65岁患者，满足《中国2型糖尿病防治指南》（2017）[8]中关于2型糖尿病的诊断标准，即空腹血糖≥7.0 mmol/L和（或）餐后2 h血糖≥11.1 mmol/L。患者需要能够使用智能手机APP进行沟通和交流，并且同时使用2种或2种以上药物进行治疗。

本研究使用掌控糖尿病APP和微信互动平台作为主要的信息化辅助工具。……
