氢吗啡酮作为佐剂用于区域麻醉的有效性及安全性研究进展

【摘要】 氢吗啡酮（Hydromorphone，HM）是强效阿片类药物，镇痛效果强，不良反应少，在临床中使用日益广泛。氢吗啡酮加入局麻药能够加快局麻药起效时间，延长局麻药作用时间，镇痛效果确切。这使得区域麻醉中使用氢吗啡酮佐剂，可能更获益。因此，本文结合其局部药理机制，综述氢吗啡酮作为局麻药佐剂在区域麻醉中使用的有效性和安全性。

[Abstract] Hydromorphone （HM） is a powerful opioid drug with strong analgesic effect and few adverse reactions， so it is widely used in clinic. Hydromorphone can accelerate the onset time of local anesthetics and prolong the action time of local anesthetics. This may make the use of Hydromorphone adjuvant in regional anesthesia more beneficial. Therefore， this paper reviews the efficacy and safety of Hydromorphone as a local anesthetic adjuvant in regional anesthesia.

氫吗啡酮（Hydromorphone，HM）又名二氢吗啡酮或双氢吗啡酮，是一种强效阿片类药物，为吗啡半合成衍生物，镇痛效果较吗啡强且不良反应少。氢吗啡酮于1924年在德国合成，1984年经美国食品药品管理局（FDA）批准应用于临床。在欧美国家，氢吗啡酮被制成多种制剂，广泛应用于各种中、重度疼痛的镇痛治疗[1]。国内氢吗啡酮静脉制剂在2013年上市以后，逐渐被应用于急慢性疼痛、围术期镇痛以及癌痛治疗等方面。

由于长期以来并没有理想的局麻药问世，而局麻药中添加阿片类等佐剂可能缩短局麻药起效时间和延长局麻药镇痛时间，达到更好的区域麻醉效果[2-3]。因此，佐剂的研究在区域麻醉中持续受到关注。而近期氢吗啡酮作为佐剂加入局麻药应用于区域麻醉的临床研究日益增多，所以本文拟综述氢吗啡酮作为佐剂在临床使用中的有效性和安全性。

1 局部应用氢吗啡酮镇痛机制
