针灸疗法联合穴位推拿治疗腰椎间盘突出症坐骨神经痛的效果分析

【摘要】 目的：分析针灸疗法联合穴位推拿在治疗腰椎间盘突出症坐骨神经痛中的效果。方法：选取2020年1月-2021年1月62例腰椎间盘突出症坐骨神经痛患者，根据治疗方法将其分为对照组（n=30）和研究组（n=32）。对照组采用针灸疗法治疗，观察组采用针灸疗法联合穴位推拿治疗。比较两组疗效及治療前后的视觉模拟疼痛评分（VAS）和改良Oswestry功能障碍指数。结果：治疗前，两组VAS评分、各维度改良Oswestry功能障碍指数比较，差异均无统计学意义（P>0.05）;治疗后，两组VAS评分、各维度改良Oswestry功能障碍指数均低于治疗前，且观察组均低于对照组，差异均有统计学意义（P<0.05）。两组总有效率均为100%。观察组的临床控制率高于对照组，有效率低于对照组，差异均有统计学意义（P<0.05）。结论：针灸疗法联合穴位推拿治疗腰椎间盘突出症坐骨神经痛，可缓解疼痛，改善功能障碍，临床疗效更佳，临床推广价值较好。

[Abstract] Objective： To analyze the effect of acupuncture therapy and acupoint massage in the treatment of lumbar disc herniation and sciatica. Method： A total of 62 patients with lumbar disc herniation and sciatica from January 2020 to January 2021 were selected， and they were divided into control group （n=30） observation group （n=32） according to different treatment methods. The control group was treated with acupuncture therapy， the observation group was treated with acupuncture therapy combined with acupoint massage. The clinical efficacy was compared between two groups， and the visual analogue pain scores （VAS） and Oswestry disability index before and after treatment were compared between two groups. Result： Before treatment， there were no significant differences in VAS scores and Oswestry disability index of all dimensions between two groups （P>0.05）; after treatment， VAS scores and Oswestry disability index of all dimensions in both groups were lower than those before treatment， and those in the observation group were lower than those in the control group， the differences were statistically significant （P<0.05）. The total effective rate in both groups were 100%. The clinical control rate of the observation group was higher than that of the control group， the effective rate was lower than that of the control group， the differences were statistically significant （P<0.05）. Conclusion： Acupuncture therapy combined with acupoint massage in the treatment of lumbar disc herniation and sciatica can relieve the patient’s pain and improve its dysfunction. The clinical effect is better and it has a good clinical promotion value.

坐骨神经痛是一种周围神经性疾病，临床上患者主要表现为在坐骨神经循行和分布区域的阵发性或持续性麻木、疼痛[1]。有研究表明，坐骨神经痛好发于40～60岁的中老年人群，发病率高达40%，其中由腰椎间盘突出引起的坐骨神经痛占90%以上[2-3]。西医治疗坐骨神经痛有多种方式，如药物治疗、手术治疗等。非甾体抗炎药、皮质醇激素等可起到缓解疼痛、对症治疗的效果，但是药物不良反应严重，患者的依从性不高;手术治疗可快速缓解患者的临床症状，但是远期疗效难以保证[4]。……
