3D-DSD技术在前牙美学缺陷重建中的作用分析

[摘要]目的：探讨三维数字化微笑设计（3D-DSD）技术在前牙美学缺陷重建治疗中的应用价值。方法：随机将61例拟行前牙美学修复患者（96颗患牙）分为两组。对照组：30例（47颗患牙），采用二维数字化微笑设计（2D-DSD）技术进行前牙美学修复;观察组：31例（49颗患牙），采用3D-DSD技术。观察两组临时修复体完成时间、全瓷修复体完成时间、修复体美国公共健康服务（USPHS）评分，粉色美学评分（PES），患者对前牙美学VAS评分的差异。结果：观察组临时修复体完成时间、全瓷修复体完成时间均低于对照组（P<0.05），修复体外形准确性USPHS评分、软组织水平、软组织轮廓PES评分高于对照组（P<0.05），患者对前牙美学满意度评分高于对照组（P<0.05）。结论：3D-DSD数字化诊疗技术可缩短前牙美学修复体设计时间，提高修复体外形准确性，更有助于改善唇、龈、齿关系，患者满意度高。

Analysis of the Effect of 3D-DSD Technique on the Reconstruction of Anterior Teeth Aesthetic Defects

Abstract： Objective To explore the application value of three-dimensional Digital Smile Design（3D-DSD） technique in the reconstruction of anterior teeth with aesthetic defects. Methods  61 patients （96 affected teeth） were randomly divided into two groups. The control group （30 cases，47 affected teeth） was treated with two-dimensional Digital Smile Design （2D-DSD） technology. The observation group （31 cases， 49 affected teeth） was treated with 3D-DSD technology. The time to complete temporary restoration， the time to complete all-ceramic restoration， the US Public Health Service （USPHS） score， the pink esthetic score （PES） and the aesthetic VAS score of anterior teeth between the two groups were observed. Results  The time of complete temporary restoration and complete all-ceramic restoration in the observation group were lower than those in the control group （P<0.05）. The USPHS Score of repair body shape accuracy， PES scores of soft tissue level and contour in the observation group was higher than that in the control group （P<0.05）. The aesthetic satisfaction of patients with anterior teeth was higher than that in the control group （P<0.05）. Conclusion 3D-DSD digital diagnosis and treatment technology can shorten the design time of anterior teeth aesthetic prosthesis， improve the accuracy of prosthesis shape， and help to improve the relationship between lip， gingiva and teeth， with high patient satisfaction.

前牙美學修复是集牙周治疗、咬合治疗、生理学、美学及预防学处理等为一体的口腔修复技术[1]。随着现代生活水平的提高，人们对前牙美学期望值不断提高，传统前牙美学修复已经不能达到患者的期望值。数字化微笑设计（Digital smile design，DSD）是目前牙齿美学的新理念和修复方法，根据国际微笑标准，采集口内牙齿长度、色泽、排列、线条、轮廓等信息，对前牙美学参数进行设计，达到完美修复体外形和笑线，更符合患者对前牙美学的需求[2-3]。目前，国内DSD应用主流为二维数字化微笑设计（Two-dimensional digital smile design，2D-DSD），但是存在无法直接生成3D图像，需要技师制作诊断蜡型，形态、色泽偏差较大等弊端。三维数字化微笑设计（Three-dimensional digital smile design，3D-DSD）利用三维数字扫描技术更精确，计算机量化设计更合理，修复效果理想，因此，更能满足现代前牙美学修复对唇、齿、龈三者协调性的要求[4-5]。……
