渭良伤科油对老年股骨粗隆间骨折术后压力性损伤的影响研究

【摘要】 目的：探讨渭良伤科油对老年股骨粗隆间骨折术后压力性损伤的影响。方法：将2018年1月-2020年6月在本院行手术治疗的140例老年股骨粗隆间骨折患者，按抽签法分为参照组和研究组，每组70例。两组患者均接受术后常规护理和局部按摩，同时参照组涂抹红花酒精，研究组涂抹渭良伤科油。比较用药2周后两组压力性损伤预防效果、反应性充血率、可逆反应性充血时间、不良反应发生情况及患者满意度。结果：研究组压力性损伤预防显效率（98.57%）高于参照组（87.14%），差异有统计学意义（P<0.05）。研究组反应性充血率（1.43%）低于参照组（12.86%），可逆反应性充血时间（4.00±1.10）h长于参照组（3.12±0.89）h，差异均有统计学意义（P<0.05）。研究组不良反应发生率（0）低于参照组（8.57%），总满意度（100%）高于参照组（90.00%），差异均有统计学意义（P<0.05）。结论：渭良伤科油可有效预防老年股骨粗隆间骨折术后压力性损伤的发生，且临床安全性、患者满意度较高，具有临床推广和应用价值。

[Abstract] Objective： To investigate the effect of Weiliang Shangke Oil on postoperative stress injury in elderly patients with intertrochanteric fracture. Method： A total of 140 elderly patients with intertrochanteric fracture who underwent surgical treatment in our hospital from January 2018 to June 2020 were divided into the reference group and study group according to drawing lots， with 70 cases in each group. Both groups received routine postoperative care and local massage， while the reference group was smeared with Safflower Alcohol， and the study group was smeared with Weiliang Shangke Oil. The prevention effect of stress injury， reactive hyperemia rate， reversible reactive hyperemia time， occurrence condition of adverse reactions and patient satisfaction were compared between the two groups after 2 weeks of treatment. Result： The effective rate of the study group was 98.57%， higher than 87.14% of the reference group， the difference was statistically significant （P<0.05）. The reactive hyperemia rate of the study group was 1.43%， lower than 12.86% of the reference group， the reversible reactive hyperemia time of the study group was （4.00±1.10）h， longer than （3.12±0.89）h of the reference group， the difference was statistically significant （P<0.05）. The incidence of adverse reactions of the study group was 0， lower than 8.57% of the reference group， and the total satisfaction of the study group was 100%， higher than 90.00% of reference group， the difference was statistically significant （P<0.05）. Conclusion： Weiliang Shangke Oil can effectively prevent the occurrence of pressure injury in elderly patients with intertrochanteric fracture， and has high clinical safety and patient satisfaction， it has clinical promotion and application value.

压力性损伤即压疮，通常是由于身体局部长时间受压造成局部组织缺血、缺氧，加之在剪切力、摩擦力等作用下致使患者局部组织坏死的一种症状，此类症状在老年、肥胖、重症监护室及长时间卧床等患者群体中较为常见[1-2]。老年股骨粗隆间骨折患者术后需长时间卧床休息，属于压力性损伤的高危群体，所以积极寻找预防患者术后压力性损伤的有效方法具有重要意义[3-4]。有学者研究指出，渭良伤科油有活血祛瘀、消肿止痛之功效，可有效预防术后长期卧床患者压疮的发生[5]。本研究选取2018年1月-2020年6月140例本院老年股骨粗隆间骨折术患者，探讨了渭良伤科油对这类患者术后压力性损伤的影响，旨在为临床寻找有效依据。现报道如下。
