强脉冲光联合胶原贴敷料治疗寻常痤疮疗效观察

[摘要]目的：探討强脉冲光联合胶原贴敷料治疗寻常痤疮的疗效。方法：选取2019年3月-2019年10月在笔者科室就诊的54例寻常痤疮患者为研究对象，根据随机数字法将患者分为对照组和观察组，每组27例。对照组采用强脉冲光治疗，治疗后予以冰敷;观察组在对照组基础上，治疗后加用胶原贴敷料外敷。比较两组患者治疗3、6周后的疗效，并记录患者治疗后皮肤潮红、干燥、瘙痒等不良反应情况。结果：治疗3周后，观察组总有效率77.78%，高于对照组的62.96%，差异有统计学意义（P<0.05）;治疗6周后，观察组总有效率92.59%，显著高于对照组的81.48%，差异有统计学意义（P<0.05）。观察组不良反应总发生率7.41%，低于对照组的18.52%，差异有统计学意义（P<0.05）。结论：强脉冲光联合胶原贴敷料治疗寻常痤疮，可显著提高疗效，减少不良反应的发生，值得临床推广应用。

Efficacy of Intense Pulsed Light Combined with Collagen Dressing in Treatment with Acne Vulgaris

Abstract：Objective  To observe the effect of intense pulsed light combined with collagen dressing in treatment with 54 patients with acne vulgaris. Methods  Total of 54 patients with acne vulgaris who were treated in our hospital from March 2019 to October 2019 were selected. According to the random number method， the patients were divided into the control group （n=27） and the observation group （n=27）. The control group was treated with intense pulsed light， and used ice compress after operation. On the basis of the treatment of the control group， the observation group was treated with collagen dressing external application. After 3 and 6 weeks of treatment， the treatment effect of the two groups of patients were compared， and the adverse reactions such as skin flushing， skin dryness， and skin pruritus were recorded after treatment. Results  After 3 weeks of treatment， the total effective rate in the observation group （77.78%） was higher than that in the control group （62.96%）， and the difference was statistically significant （P<0.05）. After 6 weeks of treatment， the total effective rate in the observation group （92.59%） was higher than that in the control group （81.48%）， and the difference was statistically significant （P<0.05）. The total incidence of adverse reactions in the observation group （7.41%） was lower than that in the control group （18.52%）， and the difference was statistically significant （P<0.05）. Conclusion  After the operation of intense pulse light， the treatment of collagen dressing external application in acne vulgaris can significantly improve the curative effect and reduce the occurrence of adverse reactions， which worthes of clinical application.

寻常痤疮是一种常见的慢性炎症性皮肤疾病，病程长、易复发，往往发生在颜面和胸背部等皮脂腺发达部位，在青少年中的发病率高达40%～60%。其主要表现为瘢痕、丘疹、粉刺及脓疱等多形态皮肤损害，易对患者的容貌外观造成影响，使患者产生自卑、焦虑的心理，降低患者自信心[1-2]。随着人们生活水平的提高和饮食习惯、生活方式的改变，寻常痤疮的发病率呈持续上升趋势，影响患者的生活质量[3]。目前，临床上应用药物治疗轻度痤疮具有良好疗效，但对于中重度痤疮的治疗存在疗效不佳、用药时间长等局限性[4]。随着医疗科技的不断发展，多种光电类仪器在痤疮治疗中起到了重要作用。强脉冲光是一种高强度的光源，通过发挥光化学作用，刺激真皮层的胶原纤维发生变化，从而达到治疗目的。其作用精准、安全无创，在痤疮治疗中具有显著疗效[5-7]。……
