信息技术在中药饮片处方审核中的应用

摘 要 目的：运用信息技术辅助处方审核，保障临床用药的合理、安全、有效。方法：对人工审方与运用信息技术辅助审方后的不合理处方进行数据统计、对比分析。结果：针对不合理处方的主要问题，结合信息技术提示功能，辅助药师处方审核，使处方不合格率下降96.30%。结论：通过HIS系统设置，运用信息技术辅助审方工作，提高了合理用药水平，保障患者用药安全。

Application of information technology in prescription review of Chinese herbal pieces*

ABSTRACT Objective： To assist prescription review using information technology and ensure the rationality， safety and effectiveness of clinical medication. Methods： The unreasonable prescription by manual prescription checking and information technology-assisted prescription checking were statistically analyzed. Results： The prescription review by pharmacists was improved by analysis of the main problems of unreasonable prescriptions in combination with the prompt function of information technology， and the unqualified rate of prescriptions was reduced by 96.30%. Conclusion： Through the setting of HIS system and the use of information technology to assist prescription examination， the level of rational drug use can be improved and the safety of drug use for patients can be ensured.

中藥饮片调剂工作由审核、调配、复核、发药等环节组成[1]，审核作为调剂工作的第一个环节，关系到调剂的质量与临床的疗效[2]。《中华人民共和国药品管理法》、《医疗机构药事管理暂行规定》、《处方管理办法》及《医疗机构处方审核规范》等相关法律、法规，阐明了处方审核的重要性，并明确了法律责任[3]。为提高处方审核的工作效率，减少人为因素的差错，本院中药房联合信息科、医教科等多部门，针对在审核过程中出现的不合理现象，于医生工作站（HIS系统）设置了不合理中药饮片处方提示功能，通过信息系统辅助中药师开展处方审核。

1 实施方法

1.1 依据来源

根据《中华人民共和国药典》（2015版）及《上海市中药饮片炮制规范》（2018版）：【性味与归经】、【用法与用量】、【注意】、附录-中药的配伍与禁忌——“十八反”“十九畏”，对本院的中药饮片品种进行整理归纳、汇总制表（表1～表3）。

1.2 调整剂量

参照2018版《上海市中药饮片炮制规范》-【用法与用量】、结合本院临床中医师用药剂量需求，对院内中药小包装的规格进行整理，使单包装的剂量符合《上海市中药饮片炮制规范》用量规定。……
