The effect of minimally invasive percutaneous plate osteosynthesis combined with ultrasound-based drug delivery on extremity fractures and postoperative swelling

[Abstract] Objective To explore the effect of minimally invasive percutaneous plate osteosynthesis combined with ultrasound-based drug delivery on limb fractures and postoperative swelling. Methods A total of sixty patients with limb fractures admitted to our hospital from June 2018 to June 2020 were selected as the research objects. They were randomly divided into two groups. Thirty cases in the control group were treated with minimally invasive percutaneous plate osteosynthesis. Thirty cases in the study group were treated with minimally invasive percutaneous plate osteosynthesis combined with ultrasound-based drug delivery. The clinical efficacy， postoperative swelling score， perioperative indicators， and total incidence of complications in the two groups were observed. Results （1）The total effective rate of the study group was 96.7%， which was significantly higher than that （63.3%） of the control group， and the difference was statistically significant （P<0.05）. （2）The swelling scores of the study group were significantly lower than those of the control group at 1， 3， 7， and 14 d after operation， and the difference was statistically significant （P<0.05）. （3）The operation time， and fracture healing time of the study group were significantly shorter than those of the control group， the intraoperative blood loss of the study group were significantly less than those of the control group， with statistically significant differences （P<0.05）. （4）Total complications incidence rate in the study group （3.3%） was significantly lower than that （23.3%） of the control group， and the difference was statistically significant （P<0.05）. Conclusion The minimally invasive percutaneous plate osteosynthesis combined with ultrasound-based drug delivery has a significant effect on extremity fractures. It can reduce postoperative swelling， shorten operation time， reduce intraoperative blood loss， reduce complications， and promote bone healing in patients， which has a certain clinical significance.

四肢骨折是目前临床上比较常见的骨外科疾病，致病原因多是由于高空坠落、车祸等因素导致，若不能及时给予治疗会导致患者骨愈合延迟、骨块缺失、四肢功能障碍、肢体畸形等并發症，致残率非常高，极大地降低了患者的生活质量。目前，对于四肢骨折的治疗仍以手术为主，传统的开放式接骨固定术治疗方法虽具有很好的疗效，但是，术后患者会出现骨邻近软组织伤口延迟愈合、骨折愈合不良及切口感染等并发症，不利于患者的预后[1]。近些年，随着我国微创技术的快速发展，微创经皮钢板接骨术被广泛应用于临床，具有创口小、术后并发症少、出血量少、患者疼痛感轻等特点，已成为临床治疗四肢骨折的理想选择[2]。临床经验表明[3-4]，手术治疗配合合理的用药能很好地提高治疗效果、改善患者的预后。既往抬高患肢给药进行消炎止痛的效果并不是很理想，超声定位透药法是一种新兴的给药方法，被广泛的应用于临床。本研究选取2018年6月至2020年6月我院收治的60例四肢骨折患者为研究对象，分组对照，探讨微创经皮钢板接骨术联合超声定位透药法对四肢骨折疗效及术后肿胀影响，现报道如下。
