腹腔镜下宫外孕手术的临床效果及对远期生育结局的影响

【摘 要】目的：分析腹腔镜下宫外孕手术临床效果及对远期生育结局的影响。方法：选取宫外孕30例进行回顾分析，其中15例接受腹腔镜下宫外孕手术治疗（观察组），另15例接受开腹手术治疗（对照组）。比较两组治疗状况。结果：观察组围术期指标低于对照组；经一年随访可知，观察组宫内妊娠情况高于对照组，未孕情况低于对照组，差异具有统计学意义；观察组宫外孕情况略低于对照组，但差异不具有统计学意义。结论：临床治疗宫外孕疾病可考虑采用腹腔镜下宫外孕手术，此手术安全性高，且患者远期生育结局非常良好，值得推广。

Clinical effect of laparoscopic ectopic pregnancy and its impact on long-term reproductive outcome

【Abstract】Objective： To analyze the clinical effect of laparoscopic ectopic pregnancy and its impact on long-term reproductive outcome. Methods： 30 cases of ectopic pregnancy were selected for retrospective analysis， of which 15 cases underwent laparoscopic ectopic pregnancy surgery （observation group） and the other 15 cases underwent open surgery （control group）. The treatment status of the two groups was compared. Results： The perioperative indexes in the observation group were lower than those in the control group； After a one-year followup， it can be seen that the intrauterine pregnancy in the observation group is higher than the control group， and the non-pregnancy is lower than the control group， the difference is statistically significant； the ectopic pregnancy in the observation group is slightly lower than the control group， but the difference is not statistically significant. Conclusion： Laparoscopic ectopic pregnancy surgery can be considered in the clinical treatment of ectopic pregnancy diseases. This operation has high safety， and the long-term reproductive outcome of patients is very good， which is worthy of promotion.

宮外孕是近年临床妇产科中较为常见的一种急腹症疾病，此疾病发病率较高，也将其称为异位妊娠，指受精卵着床位置在子宫体腔外部位，输卵管妊娠则为其中最为常见的一种，在宫外孕中占比达95%左右[1]。现临床治疗此方法主要有：1.手术治疗，2.药物治疗，3.期待治疗。近年随着腹腔镜技术在临床得到推广和应用，腹腔镜手术已在宫外孕疾病的诊治中得到认可[2]。除非生命体征不稳定需要快速进腹止血并完成手术者，需行开腹手术，输卵管妊娠手术通常在腹腔镜下完成。其优点为住院日更短，术后恢复更快，远期生育结局良好。现本研究共纳入30例宫外孕患者分组讨论此观点。

1.1 一般资料

回顾分析我院2019年4月至2020年2月收治的宫外孕患者30例，所有患者均接受B超等检查得到确诊，均签字接受手术治疗，并满足手术治疗指征。……
