从益肾健脾法论治膜性肾病经验

【摘要】膜性肾病是导致成人肾病综合征的主要原因之一。郑艳辉主任认为本病的病机以脾肾两虚为本、湿热瘀阻为标，故提出益肾健脾，佐以清热祛湿、活血通络行气的治法，标本兼顾。对于反复大量蛋白尿者合理运用激素及免疫抑制剂，取得了一定的疗效。

Director Zheng Yanhui put forward that the pathogenesis of this disease is based on spleen and kidney deficiency and dampness-heat and blood stasis as the standard.The treatment method can be summarized as follows：invigorating kidney and spleen， supplemented by clearing heat and expelling damp and promoting blood circulation and activating qi to treat both manifestation and root causes.The rational use of hormones and immunosuppressants for repeated large amounts of proteinuria has achieved certain curative effects.

膜性肾病是以肾小球基底膜弥漫性增厚，伴上皮细胞下免疫复合物沉着为主要病理特征的疾病，是导致成人肾病综合征的主要原因之一［1］。近年来，膜性肾病的发病率在我国呈逐年上升的趋势，其临床表现以水肿、大量蛋白尿、低蛋白血症、高脂血症为主，具有病程长、病情难愈、易反复发作等特点。目前西医治疗本病主要采取控制血压、调脂、利尿等对症治疗。对于病情较重、反复难愈者常使用激素及免疫抑制剂治疗，但其费用高，毒副作用大，严重影响患者生活质量及远期预后，且约1/3患者对激素及免疫抑制剂治疗的反应性差［2-3］。近年来，研究发现中医药治疗本病有其独特的优势［4］。

郑艳辉，主任中医师，师承国医大师邹燕勤教授，长期从事肾病临床工作。郑艳辉主任在治疗膜性肾病方面有其特色，笔者有幸跟随郑艳辉主任抄方学习，受益良多，现将其治疗膜性肾病经验总结如下。

1病因病机

中医学并无有关“膜性肾病”病名的记载，但根据其水肿、大量蛋白尿等临床表现，可归属于“水肿”“尿浊”“肾劳病”等范畴。……
