腰背软组织损伤的中西医综合治疗

【摘要】 目的：探讨中西医综合治疗腰背软组织损伤疼痛综合征的优势，优选临床疼痛治疗方法，减少和降低疼痛治疗的并发症。方法：在90例腰背软组织损伤综合征患者中，根据不同病情采用多种中西医治疗，主要方法有硬脊膜外阻滞+穴位+疼点、骶管阻滞+穴位+疼点、枝川疗法+按摩等。结果：近期疗效（半年内）痊愈人数占74.44%，显效人数占13.33%，好转人数占11.11%，总有效率为98.88%。远期疗效，6个月至2年随访治愈人数占87.19%，显效人数占10.26%，好转人数占1.28%，随访总有效率为98.73%。结论：在治疗腰背软组织损伤疼痛综合征中，采用中西医结合治疗的特色方法，疗效显著。中医特色治疗，可取长补短，优势互补，降低和减少并发症。

【Abstract】 Objective：To investigate the effect of integrated traditional Chinese and western medicine treatment of low back pain of soft tissue injury syndrome， optimizing clinical pain treatment and reduce complications and reduce pain therapy.Method： Ninety cases of lumbar back syndrome in patients with soft tissue injury， according to different condition using a variety of Chinese and western medicine treatment， the main methods were outside the catheter block + point + pain points， sacral canal blocking + point + pain points， branch of sichuan therapy， massage， etc.Result：The coured of the recent curative effect（half year）accounted for 74.44%， good effective rate accounted for 13.33%， effective rate accounted for 11.11%， the total effective rate was 98.88%. After 6 months to 2 years follow-up， the cured rate of long-dated curative effect was 87.19%， good effective rate was 7.69%， the effective rate was 1.28%， the total effective rate of follow-up was 98.71%.Conclusion：In the treatment of low back pain of soft tissue injury syndrome， adopt traditional Chinese and western medicine treatment method of characteristics， curative effect is distinct. The characteristics of traditional Chinese medicine treatment can complement each others advantages， and reduce complications.

腰背软组织损伤是临床上的常见病、多发病，疼痛剧烈，影响患者工作和生活，治疗不当往往转为慢性疼痛，临床治疗较为棘手[1]。该病多出现以腰背部疼痛为主的一系列症状，病因复杂。部分患者伴植物神经功能紊乱，也就是目前中医认为的“凝闪伤”、“腰肌劳损”、“坐骨神经痛”、“腰臀部肌筋膜综合征”等。本院自2010年开始采用中西医综合治疗腰背软组织损伤疼痛综合征，疗效显著，疗程短，患者满意，现报告如下。

1 资料与方法

1.1 一般资料 本组90例患者，男51例，女39例，年龄18～76岁，平均（32.2±2.6）岁；病程3 d～11年，平均（6.3±2.2）年。有外伤及劳损史者76例，本组排除骨科疾病如关节结核、骨肿瘤、骨折、骨髓炎、强直性脊椎炎、类风湿关节炎等所致的疼痛。

1.2 治疗方法

1.2.1 药物组成 （1）硬脊膜外阻滞+穴位+疼点：0.25%布比卡因5 mL+曲安缩松4 mg+维生素B1 2500 μg，维生素B 1100 mg+庆大霉素8万U+生理盐水5 mL，共配制混合液15 mL（通用液）。……
