加压输液袋替代膨宫仪在宫腔镜检查中的应用

[摘要] 目的 探讨加压输液袋替代膨宫仪在宫腔镜检查中的可行性。 方法 选择2013年1月～2014年1月本院接收并行宫腔镜检查的患者300例，随机分为两组，每组150例。A组给予膨宫仪膨宫，B组给予加压输液袋膨宫。观察比较两组膨宫的效果、宫腔镜检查清晰度、检查时间及材料成本。 结果 两组患者宫腔均处于满意的膨胀状态，视野清晰，成功完成了宫腔镜检查，B组较A组预期效果好。A组操作复杂、成本高，B组操作较简单、成本低。A组宫腔镜检查时间与B组比较，差异无统计学意义（P>0.05），B组患者膨宫液用量明显少于A组（P<0.05），两组患者均无明显不良反应发生。 结论 加压输液袋在宫腔镜检查中操作简便，安全，经济实惠，材料来源方便，显著减轻患者家庭的经济负担和社会负担，具有较好的社会效益和经济效益，值得临床推广应用。

[Abstract] Objective To investigate the feasibility of pressure infusion bags instead of uterine distention apparatus in hysteroscopy. Methods 300 patients undergoing hysteroscopy in our hospital from January 2013 to January 2014 were selected as the object，and were randomly divided into two groups，each group of 150 cases.Group A was used for uterine distention apparatus，group B was used for uterine distending pressure infusion bag.The effect uterine swelling，hysteroscopy clarity，inspection time and material costs between two groups was observed. Results Two groups of patients with uterine cavity were in a satisfactory expansion，the visual field of clear，successfully completed hysteroscopy.Group B was better than group A.The operation of group A was complex and the cost was high，and the operation of group B was simple and the cost was low.The hysteroscopy time between two groups was not statistical differenc（P>0.05）.The dosage of bentonite in Group B was significantly less than that of group A（P<0.05），There were no obvious adverse reactions between two groups. Conclusion Pressurized infusion bag in hysteroscopy has the advantages of simple operation，security，economic benefits，sources of material convenient，can significantly reduce the economic burden and social burden on patients and their families，with good social and economic benefits.It is worth of clinical application.

宫腔镜检查技术是近年来妇产科领域涌现出的一门新学科，是可以直接、全面提供宫颈和子宫腔视野的技术，能明确诊断宫腔病变的有无，已成为探究异常子宫出血原因的首选方法[1]。膨宫是宫腔镜检查成功的关键，良好的膨宫技术将使宫腔在检查中处于最佳膨胀状态，保证视野清晰，使检查安全易行[2]。以往膨宫采用膨宫仪用液体介质膨宫，水压力设定为80～100 mm Hg，流速200～400 ml/mim，每次手术后压力硅胶管需要经过浸泡消毒→高压水枪冲洗→管腔干燥→高压灭菌等程序[3]。如有连续多台宫腔镜手术，则要准备多条硅胶压力管，成本高，且易老化漏水。如采用2%戊二醛浸泡，使用前要冲洗管道，易造成污染[4]。另外，这种膨宫仪的膨宫效果差，存在空气栓塞的风险[5]。
