早期胃癌内镜下治疗术后五年生存率调查分析

【摘要】目的：分析即墨市1994-2014年早期胃癌患者内镜下治疗术后五年的观察生存率和相对生存率。方法：观察生存率用Kaplan-Meier法，相对生存率为观察生存率与同期同性别同年龄组人群寿命表中生存概率之比。结果：对即墨市1994-2014年早期胃癌患者7500例（男性4500例，女性3000例）作生存率统计。2004-2014年的3年至5年生存率显著高于1994-2004年。5年胃癌观察生存率和相对生存率随年龄增长而下降。男性l、3、5年观察生存率和相对生存率略高于女性。即墨市5年相对生存率与其它地区比较亦有差异。结论：早期胃癌内镜下治疗术后的生存率与时期、年龄、性别及地区等存在联系，胃癌生存率还与社会条件和诊治水平等有关。

【Abstract】Objective： To analyze the observed survival rate （OS） and the relative survival rate （RS） was of early stomach cancer patients after endoscopic therapy in the general population of Jimo in 1994—2014. Method： Kaplan-Meier method was used in calculating the OS， the RS was the ratio of the OS to the expected rate calculated by Jimo life table with respect to the age、sex and calender period. Results： The 3 to 10 year OS of 7500 early gastric cancer patients（male 4500，female 3000）in 2004—2014 was significantly higher than those in 1994—2003.The 5 year of RS and OS of early stomach cancer decreased with age. The 1， 3， 5year OS and RS in males were slightly higher than those in females. Conclusions： The survival rate of early gastric cancer after endoscopic therapy is related to period， age and sex， survival also related to the social condition and the health service such as the level of diagnosis and treatment.

对即墨市1994-2014年早期胃癌患者内镜下治疗术后五年的观察生存率（OS）和相对生存率（RS）作统计，描述分析全市早期胃癌患者的内镜下治疗术后生存率，并与其它地区作比较，以反映胃癌对本市居民健康的危害性。

材料与方法

1，资料来源

胃癌病例资料基于即墨市全人群肿瘤登记报告资料，死亡资料基于死亡登记报告资料，每年年底将肿瘤病例与死亡病例核对；人口资料由当地户籍科提供。生存时间截止2014年12月31日。1994-2014年胃癌患者发病共计7500例，其中男性4500例，女性3000例，剔除失访200例（2.6%）和DCO病例15例（0.02%），实际用于生存率统计的共计7285例；经病理组织学诊断的病例占69.76%。20年资料分为前后两个阶段（1994-2004年和2004-2014年）统计。……
