针刺结合穴位埋线治疗围绝经期妇女抑郁状态的临床研究

【摘要】 目的 探討针刺结合穴位埋线治疗对围绝经期妇女抑郁状态的改善效果。方法 150例围绝经期抑郁妇女， 结合就诊时间先后分为对照1组（给予针刺治疗）、对照2组（给予药物治疗）、观察组（给予针刺结合穴位埋线治疗）， 各50例。比较三组患者的治疗效果、抑郁改善情况、生存质量。结果 观察组患者治疗有效率92.0%高于对照1组72.0%及对照2组40.0%， 对照1组治疗有效率高于对照2组， 差异均具有统计学意义 （P<0.05）。治疗前， 三组患者的抑郁自评量表（SDS）评分、围绝经期生存质量量表（MENQOL）评分两两比较， 差异均无统计学意义（P>0.05）；治疗第4、8、12周， 观察组患者的SDS评分、MENQOL评分均低于对照1组及对照2组， 差异均具有统计学意义 （P<0.05）； 对照1组及对照2组患者的SDS评分、MENQOL评分比较， 差异均无统计学意义（P>0.05）。结论 针刺结合穴位埋线治疗围绝经期妇女抑郁状态相较于常规药物治疗和单纯针刺治疗效果更好， 对患者抑郁状态的改善效果更为显著， 患者生存质量明显提高， 值得临床推广。

【Abstract】 Objective To explore the improvement effect of acupuncture combined with acupoint catgut embedding in the treatment of depressive state in perimenopausal women. Methods A total of 150 perimenopausal womenin depressive state were divided by clinic time into cotrol group A （receiving acupuncture treatment）， control group B （receiving drug therapy） and observation group （receiving acupuncture combined with acupoint catgut embedding for treatment）， with 50 cases in each group. Comparison were made on therapeutic effect， depression improvement situation and quality of life in two groups. Results The observaiton group had higher treatment effective rate as 92.0% than 72.0% in control group A and 40.0% in control group B， and control group A had higher treatment effective rate than control group B. Their difference had statistical significance （P<0.05）. Before treatment， three groups had no statistically significant difference in self-rating depression scale （SDS） score， menopause-specific quality of life （MENQOL） score （P>0.05）. After 4， 8 and 12 weeks of treatment， the observation group had lower SDS and MENQOL score than control group A and control group B， and their difference had statistical significance （P<0.05）. Control group A and control group B had no statistically significant difference in SDS and MENQOL score （P>0.05）. Conclusion Combination of acupuncture and acupoint catgut embedding shows better therapeutic effect comparing with conventional drug therapy and acupuncture only， and it also can significantly improve depressive state and quality of life in patients. So it is worthy of clinical promotion.

围绝经期抑郁状态指的是患者绝经期、绝经期前、绝经后期等不同阶段均出现的抑郁情绪障碍症状， 患者表现出明显的焦虑不安、情绪低落、思维迟钝等症状， 病情严重者甚至会发展至自杀阶段， 对患者生活、工作、安全均造成严重威胁， 因此围绝经期抑郁症患者必须及早接受治疗。我国传统医学中针对围绝经期抑郁症的治疗亦有相当多的研究， 其在传统医学中被称为“郁病”、“六郁证”等。本次研究中将针刺结合穴位埋线这一传统治疗方法应用在围绝经期抑郁症患者的治疗进程中， 取得良好效果， 现报告如下。……
