夏枯草汤联合硝苯地平治疗原发性高血压的效果

[摘要]目的 探討夏枯草汤联合硝苯地平治疗原发性高血压的效果。方法 选取我院2011年3月～2016年9月收治的90例原发性高血压患者作为研究对象，随机分为观察组和对照组，每组各45例。对照组口服硝苯地平20 mg/次，早晚各1次。观察组在对照组的治疗基础上，口服夏枯草混合煎剂200 ml，早晚各1次，持续治疗120 d。常规方法检测患者治疗前后的血压和脉压，放免法测量患者治疗前后空腹情况下血浆一氧化氮（NO）和内皮素-1（ET-1）的浓度，采用ELISA法检测治疗前后患者血清中血管紧张素（Ang Ⅱ）、去甲肾上腺素（NE）和可溶性血管细胞黏附分子1（sVCAM-1）的含量。结果 观察组患者的总有效率显著高于对照组，差异有统计学意义（P<0.05）；治疗后两组患者的NO含量均降低，差异有统计学意义（P<0.05）；两组患者治疗后的血压、脉压、ET-1、Ang Ⅱ、sVCAM-1和NE含量均低于治疗前，差异有统计学意义（P<0.05）；观察组患者治疗后的NO含量高于对照组，差异有统计学意义（P<0.05）；观察组患者治疗后的血压、脉压、ET-1、Ang Ⅱ、sVCAM-1和NE含量均低于对照组，差异有统计学意义（P<0.05）。结论 夏枯草汤结合硝苯地平治疗原发性高血压能显著降低患者的血压和脉压，同时可以有效改善患者的血管功能，达到较好的治疗效果。

Effect of Prunella vulgaris Decoction combined with Nifedipine in the treatment of primary hypertension

[Abstract]Objective To investigate the effect of Prunella vulgaris Decoction combined with Nifedipine in the treatment of primary hypertension.Methods From March 2011 to September 2016，90 patients with primary hypertension treated in our hospital were selected as the subjects，and divided into the observation group and the control group randomly，45 cases in each group.The control group was treated with Nifedipine 20 mg each time，1 times in the morning and evening.The observation group was treated with mixed Decoction of Prunella vulgaris 200 ml，1 times in the morning and evening on the basis of the treatment of the control group，and treatment continued for 120 days.The blood pressure and the pulse pressure before and after treatment in patients were detected.The nitric oxide （NO） and endothelin -1 （ET-1） were detected with radioimmunoassay.The content of vascular angiotensin Ⅱ （Ang Ⅱ），norepinephrine epinephrine （NE） and soluble vascular cell adhesion molecule 1 （sVCAM-1） were detected with ELISA.Results The total effective rate of the observation group was significantly higher than that of the control group，the difference was statistically significant （P<0.05）.After the treatment，the content of NO in the two groups increased significantly （P<0.05）.The blood pressure，pulse pressure，ET-1，Ang Ⅱ，sVCAM-1 and NE after the treatment were lower than before treatment，the difference was statistically significant （P<0.05）.After the treatment，the content of NO in the observation group was higher than that in the control group （P<0.05）.The levels of blood pressure，pulse pressure，ET-1，Ang Ⅱ，sVCAM-1 and NE in the observation group were significantly lower than those in the control group （P<0.05）.Conclusion Prunella vulgaris Decoction combined with Nifedipine can significantly reduce the blood pressure and pulse pressure in patients with primary hypertension，effectively improve the vascular function of patients，and can achieve better therapeutic effect.

随着人们生活水平的提高和生活方式的转变，原发性高血压患者数量逐年攀升，已经成为威胁我国公共健康的最主要问题之一[1-2]。高血压症状严重影响机体的平衡，降低心、脑、肾等重要器官的功能，是增加心血管事件发生概率的重要因素[3-4]。治疗高血压的药物按照原理主要分为以下五类：钙通道阻滞剂（硝苯地平、尼索地平、氨氯地平等）、血管紧张素转化酶抑制（依拉普利、雷米普利等）、血管紧张素Ⅱ受体阻滞剂（撷沙坦、氯沙坦、替米沙坦等）、利尿剂（氯曝嗓、速尿等）和β受体阻滞剂（普蔡洛尔、美托洛尔、拉贝洛尔等）。其中硝苯地平应用较多，可用于合并治疗冠心病、外周血管病。夏枯草分布于中国、日本、韩国等地区[5]，具有清热解毒、消炎镇定、加快伤口愈合等功效[5-7]，其果穗可用于治疗原发性高血压和肺结核等疾病[8]。……
