下肢严重开放性骨关节创伤急诊手术中使用VSD治疗的临床效果

【摘要】 目的：探讨负压封闭引流手术（VSD）在下肢严重开放性骨关节创伤急诊手术中的应用效果。方法：选取2018年4月-2019年3月于笔者所在医院接受急诊手术治疗的108例下肢严重开放性骨关节创伤患者，随机分为观察组（VSD治疗）和对照组（常规创面处理），各54例。观察两组骨伤愈合效果及恢复情况。结果：观察组骨伤愈合优良率为92.59%，高于对照组的77.78%，差异有统计学意义（P<0.05）。观察组术后并发症发生率3.70%、换药次数（2.02±0.55）次，均低于对照组的14.81%、（9.73±1.98）次，差异均有统计学意义（P<0.05）。观察组创面愈合时间（15.72±5.16）d、住院时间（19.84±3.62）d，均短于对照组的（22.03±7.85）、（27.56±4.31）d，差异均有统计学意义（P<0.05）。结论：在下肢严重开放性骨关节创伤急诊手术中使用VSD治疗，能够有效促进患者术后恢复情况，减少并发症的发生。

Clinical Effect of VSD in Emergency Operation for Severe Open Bone and Joint Trauma of Lower Limbs/YU Hongfu. //Chinese and Foreign Medical Research， 2019， 17（28）： -22

[Abstract] Objective： To investigate the effect of vacuum sealing drainage （VSD） in emergency operation for severe open bone and joint trauma of lower limbs. Method： A total of 108 patients with severe open bone and joint trauma of lower extremities who underwent emergency surgery in our hospital from April 2018 to March 2019 were selected and divided into the observation group （VSD treatment） and the control group （routine wound treatment）， with 54 cases in each group. The healing effect of wound and recovery of the two groups were observed. Result： The excellent and good rate of wound healing in the observation group was 92.59%， higher than 77.78% of the control group， and the difference was statistically significant （P<0.05）. While the incidence of postoperative complications was 3.70% and the time of dressing changes was （2.02±0.55） times in the observation group， lower than 14.81%， （9.73±1.98） times of the control group， and the differences were statistically significant （P<0.05）. The wound healing time was （15.72±5.16） d and hospitalization time was （19.84±3.62） d in the observation group， shorter than （22.03±7.85） d and （27.56±4.31） d of the control group， and the differences were statistically significant （P<0.05）. Conclusion： The use of VSD in emergency operation for severe open bone and joint trauma of lower limbs can effectively promote the postoperative recovery of patients and reduce the occurrence of complications.

下肢嚴重开放性骨关节创伤多为意外事故所致，受伤部位会出现皮肤组织缺损、关节囊与关节面损伤及骨折，伴有剧烈的疼痛感，影响患者下肢活动，引发关节功能障碍。下肢严重开放性骨关节创伤患者需要及时接受急诊手术治疗，妥善处理创面，降低创面感染和关节感染的发生风险[1]。同时，需修复关节损伤，予以复位、固定，促进关节功能的恢复。在下肢严重开放性骨关节创伤急诊手术治疗中，提高关节腔内的清创效果，能够有效降低感染风险，确保骨伤部位的良好愈合，预防关节粘连和关节畸形等并发症的发生。在创面处理工作中，可采用VSD治疗方法，能够更好地保护创面，促进创面的良好愈合[2-3]。本研究主要探讨108例下肢严重开放性骨关节创伤患者急诊手术治疗中的创面处理方法，观察VSD的应用对于患者治疗及恢复效果的影响，报道如下。
