临床生化检验中关于血液标本放置时间对检验结果的影响

【摘要】 目的:探究血液標本放置时间在临床生化检验中对检验结果产生的影响。方法:本次试验对象均为2018年2月-2019年3月在笔者所在医院进行生化检验的95例患者,采集血液标本1、4、8 h后进行生化检验,记录并分析γ谷氨酰转肽酶、钾离子、甘油三酯、碱性磷酸酶、谷丙转氨酶、血糖及总胆红素水平。结果:标本放置1 h检测的γ谷氨酰转肽酶、钾离子、甘油三酯、碱性磷酸酶及总胆红素水平与放置4 h比较,差异均无统计学意义(P>0.05);标本放置1 h检测的谷丙转氨酶水平低于放置4 h,血糖水平高于放置4 h,差异均有统计学意义(P<0.05)。标本放置4 h检测的γ谷氨酰转肽酶、甘油三酯、碱性磷酸酶、总胆红素、血糖水平均高于放置8 h,钾离子、谷丙转氨酶水平均低于放置8 h,差异均有统计学意义(P<0.05)。结论:在临床生化检验中,血液标本放置时间对检验结果产生的影响较大,因此需合理安排检测顺序,缩短检验时间,以减少和预防检测结果的不准确性。

The Effect of Blood Specimen Placement Time on the Test Results in Clinical Biochemical Tests/LU Hongli. //Chinese and Foreign Medical Research, 2019, 17(28): -75

[Abstract] Objective: To investigate the effect of blood specimen placement time on the test results in clinical biochemical tests. Method: The subjects were selected from 95 patients who underwent biochemical tests in our hospital from February 2018 to March 2019. Biochemical tests were performed at 1, 4, and 8 h after specimen collection, and gamma-glutamyl transpeptidase, potassium ion, triglyceride, alkaline phosphatase, alanine aminotransferase, blood glucose and total bilirubin levels were recorded and analyzed. Result: The levels of gamma-glutamyl transpeptidase,potassium ion, triglyceride,alkaline phosphatase and total bilirubin were compared between 1 hour and 4 hours after specimen collection,and the differences were not statistically significant (P>0.05). But the level of alanine aminotransferase at 1 h after specimen placement was lower than that of 4 h after specimen placement,and blood glucose levels was higher than that of 4 h after specimen placement, and the differences were statistically significant (P<0.05). The levels of gamma-glutamyl transpeptidase, triglyceride, alkaline phosphatase, total bilirubin and blood glucose at 4 h after specimen placement were higher than those of 8 h after specimen placement,and the levels of potassium ion and alanine aminotransferase were lower than those of 8 h after specimen placement, and the differences were statistically significant (P<0.05). Conclusion: The blood specimen placement time has a great influence on the test results in clinical biochemical tests. Therefore, it is necessary to arrange the test sequence reasonably and shorten the test time to reduce and prevent the inaccuracy of the test results.

生化检验在临床疾病治疗和诊断中应用广泛,通常以检验结果作为辅助诊断标准,进而为临床治疗提供参考[1]。因此,在疾病诊断中进行生化检验至关重要,能够明显减少临床误诊和漏诊情况[2]。临床生化检验主要包括免疫与血液生化检验、微生物检验,用于临床诊治十分关键。同时,在一定程度上说明生化检验结果精准性至关重要,会严重影响临床医师的判断,若检验结果出现异常,极易出现误诊和漏诊情况,错失最佳治疗时间,影响治疗效果。但是血液检验科室每日检验的血液标本量较多,导致部分血液标本无法第一时间检验,需放置一段时间才可完成检验[3-4]。因此,本次对血液标本放置时间在临床生化检验中对检验结果产生的影响展开研究,并将2018年2月-2019年3月在笔者所在医院进行生化检验的95例患者纳入本次研究,报道如下。
