正畸牙周联合治疗牙周炎导致前牙移位的临床效果分析

【摘要】 目的：探究正畸、牙周聯合治疗牙周炎导致前牙扇形移位的临床效果。方法：选取2017年1月-2018年4月笔者所在医院收治的24例牙周炎导致前牙扇形移位患者作为研究对象，均予以正畸联合牙周常规治疗。对比患者治疗前后前牙覆盖、前牙覆合、前牙间牙槽骨高度、牙周袋探诊深度、探诊出血率、牙周出血指数及头影测量指标。结果：治疗1年后，24例患者前牙覆盖、前牙覆合、牙周袋探诊深度均较治疗前改善，差异均有统计学意义（P<0.05）;前牙间牙槽骨高度与治疗前比较，差异无统计学意义（P>0.05）;治疗后，24例患者探诊出血率及牙周出血指数均明显低于治疗前，差异均有统计学意义（P<0.05）;治疗后U1-Ptm、U1-SN、Ls-EP均较治疗前减小，NLA较治疗前增加，差异均有统计学意义（P<0.05）。结论：正畸联合牙周治疗可以显著改善患者的牙周袋探诊深度及探诊出血率，同时减少前牙覆合覆盖，减小上前牙的突度和上唇的突度，增大鼻唇角，改善患者面型和唇齿关系，因此具备临床推广价值。

【Abstract】 Objective：To explore the clinical effect of orthodontic and periodontal combined treatment on anterior teeth fan displacement caused by periodontitis.Method：24 cases of anterior teeth fan displacement caused by periodontitis in our hospital from January 2017 to April 2018 were selected as the subjects of the observation.All patients received routine treatment of orthodontic combined with periodontal.To compare overjet and overbite of anterior teeth，height of alveolar bone between anterior teeth，probing depth of periodontal pocket，probing bleeding rate，periodontal hemorrhage index and cephalometric measurements before and after treatment.Result：One year after treatment，the overjet and overbite of anterior teeth and probing depth of periodontal pocket of 24 patients were improved compared with that before treatment，and the differences were statistically significant（P<0.05）.The height of alveolar bone between anterior teeth compared with that before treatment，the difference was not statistically significant（P>0.05）.After treatment，the probing bleeding rate and periodontal hemorrhage index of 24 patients were significantly lower than those before treatment，and the differences were statistically significant（P<0.05）.After treatment，U1-Ptm，U1-SN，NLA，Ls-EP were all reduced compared with those before treatment，while NLA increased compared with that before treatment，and the differences were statistically significant（P<0.05）.Conclusion：Orthodontics combined with periodontal therapy can significantly improve probing depth of periodontal pocket and probing bleeding rate.The overjet and overbite of anterior teeth are reduced.And the protrusion of anterior upper teeth and upper lip are reduced，and the nasolabial angle is increased.The patientsappearance and the lip and tooth relations were improved.Therefore，this treatment method has clinical promotion value.

牙周炎是一种可对牙周膜、牙龈及牙槽骨等部位造成较大损害的临床常见慢性疾病，在我国具有较高的发病率[1]。随着牙周炎病情的恶化，牙周组织会被严重破坏，牙槽骨吸收程度逐渐加深，最终引发前牙变形、散开等，使前牙形成扇型移位[2]。如果不及时进行有效的医疗干预，患者的牙齿可能会脱落，影响面部的美观度及正常的咀嚼功能，给生活带来诸多不良影响[3]。目前，牙周炎导致前牙扇形移位的主要治疗方法为正畸治疗，在临床具有较为广泛的认可度。鉴于此，本文就正畸、牙周联合治疗牙周炎导致前牙扇形移位的临床效果展开如下探讨。
