【摘要】 目的:觀察老年高血压左室肥厚(LVH)与非左室肥厚(NLVH)患者在运动心肺试验中各检测指标的变化,探讨左室质量指数(LVMI)与各指标相关性。方法:纳入116例老年高血压患者,根据LVMI水平分为NLVH组(n=74)及LVH组(n=42),测量两组无氧阈(AT)时摄氧量(VO2)、代谢当量(MET)、氧脉搏(VO2/HR)、相对摄氧量(VO2/kg),以及最大运动量(max)时摄氧量(VO2)、代谢当量(MET)、氧脉搏(VO2/HR)、相对摄氧量(VO2/kg),比较两组各数值的差异性及与LVMI相关性。结果:LVH组达到AT值时VO2显著低于NLVH组(P<0.05);两组达到AT值时MET、VO2/HR及VO2/kg比较,差异均无统计学意义(P>0.05)。LVH组达到max值时VO2、MET显著低于NLVH组(P<0.05);两组达到max值时VO2/HR及VO2/kg比较,差异均无统计学意义(P>0.05)。Pearson相关分析显示,LVH组VO2 AT、VO2 max、MET AT、MET max、VO2/kg AT、VO2/kg max与LVMI均呈负相关(r=-0.312、-0.376、-0.390、-0.475、-0.370、-0.492,P<0.05)。结论:老年高血压合并LVH患者出现VO2 AT、VO2 max、MET max下降,表明运动耐量受损,摄氧能力下降。

【Abstract】 Objective:To observe the changes of the difference of the indexes of exercise cardiopulmonary test in elderly patients with hypertension complicated with left ventricular hypertrophy(LVH) and non-left ventricular hypertrophy(NLVH),and to investigate the correlation between left ventricular mass index(LVMI) and various indexes.Method:A total of 116 elderly patients with hypertension were divided into the NLVH group(n=74) and the LVH group(n=42) according to the level of LVMI.The oxygen uptake(VO2),metabolic equivalent(MET),oxygen pulse(VO2/HR),relative oxygen uptake(VO2/kg) at anaerobic threshold(AT) and the oxygen uptake(VO2),metabolic equivalent(MET),oxygen pulse(VO2/HR),relative oxygen uptake(VO2/kg) at maximal exercise(max) in both groups were measured.The differences between the two groups of indexes and the correlation with LVMI were compared.Result:The level of VO2 at AT in the LHV group was significantly lower than that of the NLVH group(P<0.05).While the levels of MET,VO2/HR and VO2/kg at AT were compared between the two groups,and the differences were not statistically significant(P>0.05).The levels of VO2 and MET at max in the LVH group were significantly lower than those of the NLVH group(P<0.05).While the levels of VO2/HR and VO2/kg were compared between the two groups,and the differences were not statistically significant(P>0.05).Pearson correlation analysis showed that VO2 AT,VO2 max,MET AT,MET max,VO2/kg AT,VO2/kg max in the LHV group were negatively correlated with LVMI(r=-0.312,-0.376,-0.390,-0.475,-0.370,-0.492,P<0.05).Conclusion:The levels of VO2 AT,VO2 max and MET max were decreased in elderly patients with hypertension complicated with LVH,indicating impaired exercise tolerance and decreased oxygen uptake.

目前公認原发性高血压病(essential hypertension,EH)是最常见的心血管疾病,发病随着年龄的增长而逐年增加。根据2017年中国心血管疾病报告显示,目前中国EH患病人数达2.7亿[1]。有研究报道,EH患者超过50%合并左室肥厚(left ventricular hypertrophy,LVH)[2]。国内临床研究证实,无冠状动脉病变的EH合并LVH患者存在心肌微血管病变,主要是通过对比心脏超声图(MCE)、心肌声学造影检查等证实,临床上比较难以开展。本研究通过心肺运动方式,根据老年EH合并LVH患者运动负荷状态下的机体代谢参数的变化,测量患者的运动能力及与LVMI的关系,间接评价心脏储备能力,以期指导临床的治疗。
1 资料与方法
1.1 一般资料
采用回顾性研究分析方法,收集2016年6月-2018年6月在笔者所在医院首诊老年EH患者116例,女48例,男68例。纳入标准:(1)年龄>60岁;(2)符合2018年《中国高血压防治指南》中EH诊断标准[3]。排除标准:(1)各类冠心病;(2)糖尿病;(3)曾有或目前合并纽约心功能分级≥Ⅲ级;(4)慢性肺疾病;(5)瓣膜性心脏病;(6)心律失常、贫血、甲状腺功能异常;(7)合并神经、肌肉、关节病变等影响运动心肺功能检测或不能完成检查。所……
