【摘要】 目的:探究振幅整合腦电图用于重度高胆红素血症新生儿体格发育水平和智力发育水平的评估价值。方法:选择笔者所在医院2016年1月-2017年12月收治的600例重度高胆红素血症新生儿作为研究对象,入院后,对所有患儿行脑功能监护仪监测,并根据监测振幅整合脑电图结果将患儿分为异常组(n=244)和正常组(n=356)。对两组随访1年,对比体重、身长、头围、智力发育指数及心理运动发育指数。结果:正常组体重(9.6±0.8)kg、身长(75.3±0.6)cm、头围(46.4±1.3)cm及智力发育指数(92.1±7.0)分、心理运动发育指数(94.6±5.5)分,均显著优于异常组的(8.4±0.5)kg、(68.1±0.6)cm、(44.1±1.0)cm、(85.4±6.3)分、(87.9±5.2)分,差异均有统计学意义(P<0.05)。结论:振幅整合脑电图异常,可提示重度高胆红素血症新生儿体格发育水平及智力发育水平不佳,因此,可作为临床评估重度高胆红素血症新生儿体格发育水平及智力发育水平的重要
依据。

【Abstract】 Objective:To explore the value of amplitude integrated electroencephalogram in the assessment of the level of physical development and the level of intelligence development in the newborn with severe hyperbilirubinemia.Method:A total of 600 newborn with severe hyperbilirubinemia admitted to our hospital from January 2016 to December 2017 were enrolled in the study.After admission,all newborn were monitored by brain function monitor.The newborn were divided into the abnormal group(n=244) and the normal group(n=356) according to the results of amplitude integrated electroencephalogram.The two groups were followed up for one year to compare the body weight,body length,head circumference and intelligence development index and psychomotor development index.Result:In the normal group,the body weight was (9.6±0.8)kg,the body length was (75.3±0.6)cm,the head circumference was (46.4±1.3)cm,
the intelligence development index was (92.1±7.0)points and the psychomotor development index was (94.6±5.5)points,which were significantly better than those of the abnormal group (8.4±0.5)kg,(68.1±0.6)cm,(44.1±1.0)cm,(85.4±6.3)points and (87.9±5.2)points,and the differences were statistically significant(P<0.05).Conclusion:Amplitude integrated electroencephalogram abnormality may be suggested that the level of physical development and the level of intelligence development of newborn with severe hyperbilirubinemia is not good.Therefore,it can be used as an important basis for clinical evaluation of the level of physical development and the level of intelligence development of newborn with severe hyperbilirubinemia.

高胆红素血症即黄疸,是新生儿出生2~3 d后最常见疾病之一,主要因患儿机体胆红素代谢障碍引起血清内胆红素浓度升高所致。本病不仅会引发患儿出现巩膜、皮肤等组织黄染、腹痛、腹胀、尿和粪色泽改变等癥状,同时还会导致脑细胞产生间接性神经损伤,从而引发发育障碍、智力缺陷等并发症[1]。为改善患者预后,需要临床尽早采取有效的检测手段对患儿脑功能进行监测,以便临床及时采取相应方案对患儿进行治疗和干预[2]。振幅整合脑电图是一种以过滤压缩的脑电图为基础,利用脑功能监测仪记录得出的图谱,是新生儿重症监护病房对患儿脑功能进行监测的有效辅助手段[3]。本文主要探究振幅整合脑电图用于重度高胆红素血症新生儿体格发育水平和智力发育水平评估的价值,报道如下。……
