Clinical Analysis of the Effect and Complication of Sclerotherapy for Venous Malformation of the Lips in Children

Abstract：Objective  To explore the clinical effects and complications of sclerotherapy for lip venous malformations in children. Methods  Seven children with venous malformation of the lips were treated with polidocanol foam first combined with absolute ethanol，the efficacy and complicationgs were analyzed. Results  All children in this group achieved good therapeutic effects， but complications such as mucosal ulcers and swelling of the operation area also occurred. Conclusion  The application of polidocanol foam combined with absolute ethanol in children with venous malformations of the lips has a good effect， but there are certain risks.

静脉畸形（Venous maifarmationg，VM）是胚胎发生过程中静脉系统发育受阻所致的血管结构畸形，治疗手段包括手术切除、血管内硬化治疗、激光治疗及铜针埋置[1]等，目前，血管内硬化治疗是治疗静脉畸形的主要方法[2]。静脉畸形多发生在头面部[3]，唇部静脉畸形可造成外观损毁及功能障碍，尤其是儿童唇部组织菲薄，治疗更为困难，且风险较大。近年来，笔者科室采用聚多卡醇泡沫联合无水乙醇硬化治疗7例唇部静脉畸形患儿，均取得了良好的臨床效果，现报道如下。

1  资料和方法

1.1 一般资料：选取2016年12月-2020年10月泉州市儿童医院整形美容外科收治的7例唇部静脉畸形的患儿为研究对象，采用聚多卡醇泡沫联合无水乙醇血管内硬化治疗。本组7例患儿，其中男5例，女2例，年龄4个月至9岁，平均3岁7个月，体重6～30kg，平均12.1kg，位于上唇者4例，位于下唇者3例，1例合并鼻唇沟静脉畸形;2例合并舌体静脉畸形，其中1例除唇部、舌体外，同时合并有面颈部静脉畸形，1例合并蚕豆病。所有患儿均完善术前检查，并询问乙醇过敏史，行彩超及MRI平扫+增强检查，明确病灶性质及范围，7例患儿均符合静脉畸形诊断标准[4]。

1.2 纳入标准：①病史资料完整;②不合并毛细血管、淋巴管畸形等其他脉管畸形;③既往无治疗史。

1.3 治疗方法：术前参照MRI检查所示病灶范围，应用彩超引导标注病变范围。所有患儿均采用全身麻醉，术野常规消毒、铺巾，充分暴露病灶，术中再次彩超引导下采用双针法[5]多点注射，每两点间隔约1cm，注射间隔大于5min。……
