精神专科医院2017—2019年二类精神药品门诊使用情况分析

摘 要 目的：了解精神专科医院2017—2019年二类精神药品门诊使用情况，为临床合理用药提供参考。方法：采用世界卫生组织（WHO）推荐的日限定剂量为指标的分析方法，对上海市松江区精神卫生中心2017—2019年二类精神药品的销售金额、用药频度（DDDs）、日均费用（DDC）进行统计分析，评估二类精神药品的用药合理性。结果：二类精神药品使用金额和用药频度逐年上升，右佐匹克隆销售金额最高，氯硝西泮用量最大，DDDs排名前3的分别是氯硝西泮、右佐匹克隆和阿普唑仑。苯二氮？类药物DDC较小，非苯二氮？类DDC较高，右佐匹克隆的药品排序比（B/A）等于1，同步性好。结论：我院二类精神药品结构合理，临床上非苯二氮？类药品已逐步取代苯二氮？类。精神科药师需要关注二类精神药品用药疗程，促进临床合理用药。

Drug utilization study of psychotropic drugs of category Ⅱ in outpatients in a psychiatric hospital from 2017 to 2019

ABSTRACT Objective： To investigate the application of psychotropic drugs of category Ⅱ in outpatients in Shanghai Songjiang Mental Health Center from 2017 to 2019， and to provide reference for clinical rational use of drugs. Methods： By using the research index-defined daily dose （DDD） recommended by the World Health Organization （WHO）， the sales amount， drug use frequency （DDDs） and defined daily cost （DDC） of category Ⅱ psychotropic drugs in outpatients during 2017-2019 were statistically analyzed. Results： The use amount and frequency of category Ⅱ psychotropic drugs increased year by year， with the sales amount of eszopiclone the highestand the dosage of clonazepam the largest， and the top three DDDs were clonazepam， eszopiclone and alprazolam， respectively. The DDC of benzodiazepine （BZD） drug was smaller than that of nonbenzodiazepine（NBZD） drug， and the drug sequence ratio （B/A） of eszopiclone was equal to 1. Conclusion： The structure of category Ⅱpsychotropic drugs in our hospital is reasonable， and NBZD drugs have gradually replaced BZD drugs in clinic. Psychiatrists need to pay attention to the treatment course of category Ⅱ psychotropic drugs and promote the rational clinical use of drugs.

二類精神药品，如巴比妥类、苯二氮？类及新型非苯二氮？类等，有镇静、催眠、抗焦虑作用，在精神专科临床应用普遍[1]。但因其属于特殊药品，在发挥治疗作用的同时若使用不当极易产生依赖性，更需要关注其临床使用的规范性和合理性，故笔者对本院2017—2019年二类精神药品门诊使用情况进行统计分析，为促使临床合理用药提供参考。

1 资料与方法

资料来源于我院医院信息管理系统（HIS）提供的2017—2019年门诊二类精神药品出库数据，包括药品名称、规格、数量、价格、销售金额等。

应用Excel软件对我院门诊二类精神药品使用情况进行统计。……
