延续性护理实践中存在问题及应对策略分析

摘  要：目的  了解延续性护理实践中存在的问题及其应对策略。方法  采用半结构式访谈法，对11名护理人员进行深入访谈，并采用内容分析法分析访谈资料。结果  延续性护理实践中存在的问题包括延续性护理供需不平衡、质量标准缺乏和提供者以护士为主，应对策略包括以患者需求为导向开展延续性护理、制定延续性护理的质量标准和加强延续性护理的多学科团队建设。结论  医疗机构需了解患者对延续性护理的需求，分析延续性护理存在的问题，结合相应的应对策略，发展延续性护理模式，为继续开展延续性护理提供方法指导。

Analysis of Problems and Coping Strategies in Continuous Nursing Practice

Abstract： Objective  To understand the problems and coping strategies in the practice of continuous nursing. Methods  11 nurses were interviewed by semi-structured interview， and the data were analyzed by content analysis. Results the problems existing in the practice of continuous nursing include the imbalance of supply and demand of continuous nursing， the lack of quality standards and the fact that the providers are mainly nurses. The coping strategies include carrying out continuous nursing based on the needs of patients， formulating quality standards of continuous nursing and strengthening the construction of multidisciplinary team of continuous nursing. Conclusion  Medical institutions need to understand the needs of patients for continuous nursing， analyze the problems existing in continuous nursing， and develop continuous nursing mode combined with corresponding coping strategies， so as to provide method guidance for continuous nursing.

隨着分级诊疗制度建设的推进，患者在不同医疗机构间转诊，护理服务的连续性面临更大的挑战。延续性护理是指“患者发生在不同医疗机构转移或不同照护层次转变，提供连续性、协调性和整体性的照护服务，以维护患者健康或满足其健康需求”。近年来，我国鼓励医疗机构为出院患者提供延续性护理，各级医疗机构也开展了延续性护理工作，但效果并不理想。该研究旨在通过访谈护理人员，了解延续性护理实践中存的问题及应对策略，为相关行政管理部门和医疗机构继续推进延续性护理工作提供参考。

1  研究对象与方法

1.1 研究对象
