摘 要:目的 探讨经皮穴位电刺激(Transcutaneous electrical acupoint stimulation,TEAS)对人工髋关节置换术老年患者体温及循环系统影响。方法 2018年5月~2019年8月选取在泰安市中医医院行单侧人工全髋关节置换术的老年患者60例,将入选病例采用数字表抽取法分组,对照组30例应用常规麻醉方式,观察组30例在常规麻醉基础上,取经皮穴位电刺激法加用,观察两组TEAS开始前(T0)、TEAS 30 min时(T1)、蛛网膜下腔麻醉完成后(T2)、手术切皮前(T3)、手术开始后1 h(T4)、手术结束时(T5)鼻咽温度、心率、平均动脉压水平。结果 观察组鼻咽温度在T4、T5时点高于对照组(P<0.05)。两组平均动脉压、心率波动无差异(P>0.05)。结论 老年人工髋关节置换手术患者应用经皮穴位电刺激技术,可促体温稳定,减轻循环波动。

Abstract: Objective To investigate the effect of transcutaneous electrical acupoint stimulation (TEAS) on the body temperature and circulatory system of elderly patients undergoing artificial hip replacement. Methods From May 2018 to August 2019, 60 elderly patients undergoing unilateral total hip arthroplasty were selected. The selected cases were grouped by digital table extraction. 30 cases in the control group received conventional anesthesia, and 30 cases in the observation group On the basis of conventional anesthesia, percutaneous electrical acupoint stimulation was used to observe the two groups before the start of TEAS(T0), at 30 minutes of TEAS(T1), after the completion of subarachnoid anesthesia(T2), before surgical skin incision(T3), One hour after the start of the operation (T4)and at the end of the operation(T5) nasopharyngeal temperature, heart rate, mean arterial pressure level. Results The nasopharyngeal temperature of the observation group was higher than that of the control group at T4 and T5(P<0.05). There was no difference in mean arterial pressure and heart rate fluctuation between the two groups(P>0.05). Conclusion The application of percutaneous electrical acupoint stimulation to elderly patients undergoing hip replacement surgery can promote body temperature stability and reduce circulatory fluctuations.

循环稳定和体温恒定是人体进行正常生理功能的前提。随年龄增长,就临床收治的老年人而言,各器官功能也表现为渐趋减退的状态,手术中更易发生循环波动和低体温[1]。因此,维持老年病人手术期间循环及体温稳定也就更加引起了人们的关注。近年来,随着人口老龄化社会进程加剧,人工髋关节置换术在临床已广泛开展,如何在提高手术效果的同时,保障老年患者围术期安全是普遍关注的问题[2]。本研究旨在探究经皮穴位电刺激(TEAS)对老年下肢手術患者人循环及体温的影响,现讨论如下。
1 资料与方法
1.1 一般资料
报经伦理学组织委员会批准,对2018年5月~2019年8月在泰安市中医医院行单侧人工全髋关节置换术的老年患者60例。……
