雷火灸对产后子宫复旧的效果评价

摘 要 目的：探讨雷火灸对产后子宫复旧的治疗效果。方法：选择2018年10月—2020年4月在江西省百佳艾玛妇产医院分娩并接受产后子宫复旧治疗的产妇56例，按照随机数字表对照法分为两组，每组28例。对照组采用生理康复治疗，观察组联合雷火灸康复治疗，1 次/d，治疗42 d。观察两组治疗指标评分、疗效、凝血功能和子宫三径之和。结果：观察组恶露量、宫底高度、血紫黯有块、宫缩疼痛评分和总评分均低于对照组（P<0.05）。观察组治疗总有效率为96.43%（27/28），高于对照组的67.86%（19/28，P<0.05）。观察组凝血功能改善效果优于对照组（P<0.05）；且观察组第3天和第42天时的子宫三径之和均低于对照组（P<0.05）。结论：雷火灸对产后子宫复旧治疗效果明显，能够改善患者临床症状，减少恶露量，恢复宫底高度，减轻患者疼痛，值得临床推广应用。

Evaluation of effect of thunder fire moxibustion on postpartum uterine involution

ABSTTRACT Objective： To explore the effect of thunder fire moxibustion in the treatment of postpartum uterine involution. Methods： Fifty-six parturients who delivered and received postpartum uterine involution in Baijia Emma Obstetrics and Gynecology Hospital， Jiangxi from October 2018 to April 2020 were selected and according to the random number table comparison method， divided into two groups with 28 cases in each group. The control group was treated with physiological rehabilitation therapy， and the observation group was combined with thunder fire moxibustion rehabilitation therapy， 1 time/d， for 42 d. The treatment index score， curative effect， coagulation function and the sum of three uterine diameters of the two groups were observed. Results： The amount of lochia， height of uterine fundus， blood purple mass， uterine contraction pain and total score of the observation group were lower than those of the control group（P<0.05）. The total effective rate of the observation group was 96.43% （27/28）， which was higher than that of the control group 67.86%（19/28， P<0.05）. The improvement effect of coagulation function of the observation group was better than that of the control group（P<0.05）； and the sum of the three diameters of the uterus on the 3rd and 42nd day of the observation group was lower than that of the control group（P<0.05）. Conclusion： Thunder fire moxibustion has obvious effects in the treatment of postpartum uterine involution， which can improve the clinical symptoms of patients， reduce the amount of lochia， restore the height of the uterine fundus， and relieve the pain of the patient， which is worthy of clinical application.

子宫复旧不良是指产妇在分娩后，血性恶露持续时间在10 d以上，而且还表现为量多、臭味，该病症是妇产科常见病癥[1]。现阶段，剖宫产率较高，产妇经剖宫产分娩后，子宫收缩不良，子宫复旧受到影响，容易发生宫腔内积血[2]，临床上需要积极治疗，减少宫腔感染、贫血等情况的发生[3]。江西省百佳艾玛妇产医院引入中医雷火灸疗法，对患者进行积极干预，汇报如下：

1 资料与方法

1.1 一般资料

选择2018年10月—2020年4月在江西省百佳艾玛妇产医院分娩并接受产后子宫复旧治疗的产妇56例，均符合子宫复旧不良诊断标准[4]，且经B超正点显示宫内有残留物，宫腔内有液性暗区。按照随机数字表对照法分为两组，每组28例。对照组年龄为22～34岁，平均年龄为（31.14±1.44）岁；均为足月妊娠，孕周为37～41周，平均孕周为（38.04±0.66）周；经阴道分娩20例，经剖宫产分娩8例。观察组年龄为23～34岁，平均年龄为（31.35±1.62）岁；均为足月妊娠，孕周为37～41周，平均孕周为（38.12±0.71）周；经阴道分娩21例，经剖宫产分娩7例。……
