摘 要 本研究选取24例呼吸疾病住院患者进行DPI吸入装置问卷调查,根据调查结果,临床药师对患者进行用药宣教并对宣教前后患者DPI吸入装置使用知晓度情况变化进行分析。宣教前患者使用DPI吸入装置错误率75%,主要存在的问题包括吸入前未呼气、吸入时未保持气流平稳连续以及吸入后未屏气等。临床药师用药宣教后,患者使用DPI吸入装置的错误率降至20.83%,有了显著的降低。

Effect of medication education of DPI inhalation device for patients with respiratory diseases

ABSTRACT Twenty-four inpatients with respiratory disease were selected for questionnaire survey of DPI inhalation device. Clinical pharmacists conducted medication education for the patients and analyzed the change of their awareness of DPI inhalation device before and after the education based on the survey result. The error rate of patients using DPI inhalation device accounted for 75% before education, in which the main problems included failure to exhale before inhalation, failure to maintain smooth and continuous airflow during inhalation and failure to hold breath after inhalation. The error rate could decrease to 20.83% with a significant reduction under the clinical pharmacists clinical medication education.

以慢性阻塞性肺疾病(chronic obstructive pulmonary disease, COPD)和支气管哮喘(哮喘)为代表的慢性气道疾病是我国现阶段疾病防控的一大挑战。《慢性阻塞性肺疾病全球倡议》(the Global Initiative for Chronic Obstructive Lung Disease, GOLD)、全球支气管哮喘防治倡议(the Global Initiative for Asthma, GINA)和我国指南均一致推荐吸入疗法作为COPD和哮喘患者的一线基础治疗方法[1]。然而由于患者经常使用吸入剂不当而导致疾病控制不佳及治疗费用的增加,或需要增加药物使用量来进行预防性治疗,这是COPD及哮喘中最普遍存在的问题[2]。有研究表明[3-4],在长期使用吸入装置治疗的COPD和哮喘患者,吸入装置使用的错误率极高,甚至很多相关的医务人员不能做到正确地教育患者如何使用吸入装置。造成这一现象的原因很多,其中正确给药是至关重要的一环,其主要内容包括给药过程中患者依从性的差异、吸入技术的差异、给药者的给药手法的差异以及吸入与给药的配合的差异等[5]。
