体位管理联合无痛分娩对初产妇结局的影响

摘要：目的：深入研究和探讨使用无痛分娩联合体位管理对初产妇分娩结局产生的影响。方法：本研究所选择的案例为2020年3月-2021年3月期间我院所接诊的120例初产妇案例资料，并且按照随机分组的方式平均将其分为了观察组和对照组，且每组患者的数量为60例。对照组患者采用无痛分娩方式，观察组患者则在患者自愿的情况下采用无痛分娩联合体位管理的方式。在生产过程中采用自由舒适的体位直到宫口全开，之后指导产妇用力，直到顺利分娩。对比两组患者生产产程时间、分娩方式等相关指导。结果：观察组患者在采用无痛分娩联合体位管理进行分娩指导过后第一二产程的时间明显短于对照组，即（p<0.05），同时自然分娩的概率明显高于对照组，两组对比具有统计学意义。结论：采用无痛分娩联合体位管理的方式可以帮助初产妇更好的掌握用力方式，同时也可以加速产程，降低了出现剖宫产的概率，同时也提高了引导顺产分娩的概率，是有效提高母婴健康的重要举措，具有极其重要的临床推广意义和价值。

Abstract：Objective：To study the effect of painless delivery combined with body position management on the delivery outcome of primipara. Methods：the cases selected in this study were 120 cases of primipara admitted in our hospital from March 2020 to March 2021，and they were divided into observation group and control group according to random grouping，and the number of patients in each group was 60. The patients in the control group were given painless delivery，while the patients in the observation group were given painless delivery combined with body position management on a voluntary basis. In the process of production，use a free and comfortable position until the uterine mouth is fully opened，and then guide the maternal to exert force until the smooth delivery. The time of labor process and mode of delivery were compared between the two groups. Results：the time of the first and second stage of labor in the observation group was significantly shorter than that in the control group（P < 0.05），the probability of spontaneous delivery was significantly higher than that of the control group，and the comparison between the two groups was statistically significant. Conclusion：the method of painless delivery combined with body position management can help primipara better master the force mode，accelerate the labor process，reduce the probability of cesarean section，and improve the probability of guiding spontaneous delivery，which is an effective way to improve the health of mothers and infants Important measures have extremely important clinical significance and value.

引言：

近些年来伴随着医疗条件的不断改革与发展，无痛分娩技术日益成熟，并且被广泛应用到了临床推广和实践的过程当中，所谓无痛分娩，实际上就是指在切实保障产妇以及胎儿安全的前提之下，通过采用科学正确的用药方式，从而在确保不影响宫缩的状况下有效阻断了分娩时候的痛觉神经传递感受，最终目的是为了切实减轻产妇分娩时的疼痛感。但是，无痛分娩这一手段因为是在抑制必要的疼痛反射，从而减轻宫缩疼痛程度，所以也就会在一定程度上影响和减慢宫口扩张的速度，同时也会在一定程度上增加胎位不正发生的概率，严重的影响产程。从目前研究的實际进展来看，本研究所选择60例观察组患者采用无痛分娩联合体位管理方法之后，取得了相对较好的临床效果，促进了自然分娩的概率，同时也降低了出现剖宫产的可能，帮助患者提升了无痛分娩率，现报道如下。
