【摘要】目的 探讨Herbert螺钉治疗MasonⅢ型桡骨小头骨折的临床疗效。方法2010年8月～2014年8月我院收治的24例MasonⅢ型桡骨小头骨折患者采用了切开复位Herbert螺钉内固定术，对其中18例随访资料完整的病例进行分析。结果 18例获得随访，平均随访（23．5±2．6）个月，所有患者骨折愈合良好，平均愈合时间（5．8±0．6）个月，无关节不稳定及骨不连发生。按Metaizeau等的肘关节功能后期疗效标准分为：良好12例，较好5例，一般1例，临床疗效满意。结论 Herbet螺钉内固定治疗桡骨小头骨折的临床疗效良好，术后可早期进行功能锻炼，恢复关节功能。

Objective To evaluate the clinical effect of Herbert screwsin internal fixation of the Mason Ⅲ type of radial head fractures．Methods From August 2010 to August 2014，24 cases of Masson Ⅲ type fracture of the radial head fracture patients were treated by open reduction and Herbert screw internal fixation． Results 18 cases received follow-up，with an average follow-up（23．5±2．6） months． All patients with fracture healing was good， the average healing time （5．8±0．6） months，no joint instability and bone nonunion． According to the elbow function late curative effect standard，12 cases were good，5 cases were better，generally in 1 case，and clinical curative effect was satisfied．Conclusion Internal fixation of Herbert screws can achieve satisfactory effect for Masson Ⅲ type of radial head fracture with advantages as rigid internal fixation and early functional exercises．

桡骨小头骨折是临床典型肘部骨折病症之一，目前在对桡骨小头骨折的治疗方法上尚存在一定的分歧与争议，临床中所应用的治疗方案包括保守治疗、小头置换术、小头切除术、以及切开复位内固定等［1］。我院于2010年8月～2014年8月收治的24例Mason Ⅲ型桡骨小头骨折采取了Herbert 螺钉内固定术，术后取得良好的临床效果，现对18例随访资料完整的病例回访报告如下。

1 资料与方法

1.1 一般资料

选取2010年8月～2014年8月，我院骨科所收治Mason Ⅲ型桡骨小头骨折患者共计24例，其中18例获得完整随访资料。其中男性10例，女性8例，年龄30～65岁，平均年龄（41.8±3.9）岁，左侧10例，右侧8例，坠落伤2例，车祸伤5例，自己摔伤11例；伤后均表现出肘关节外侧肿胀、疼痛、旋后障碍，无神经损伤表现，无张力性水疱，并摄片及CT三维重建确定骨折类型。

1.2 手术方法
