肱骨远端AO—C型骨折关节内粉碎性骨折的临床疗效研究

[摘要] 目的 探讨肱骨远端AO-C型骨折关节内粉碎性骨折的临床疗效。 方法 方便选择该院2016年4月—2017年4月收治的56例肱骨远端AO-C型骨折关节内粉碎性骨折患者，根据治疗方式的不同对30例患者进行张力带法固定治疗，对26例应用Y形钢板固定治疗。统计治疗效果以及不良反应发生情况。 结果 双张力带组治疗总有效率为96.67%高于Y形钢板组的80.77%（χ2=8.976，P<0.05）。双张力带组治疗满意率为100.00%，高于Y形钢板组的92.31%（χ2=9.042，P<0.05）。双张力带组不良反应发生率23.08%比双张力带组的10.00%要高（χ2=7.485，P<0.05）。结论 对于肱骨远端C型骨折关节内粉碎性骨折患者实施内固定治疗，双张力带治疗效果在各方面均有优势，不仅操作简单、可靠安全性高，适合各类型骨折患者，而Y形钢板适合C1骨折患者开展治疗。

Research on Curative Effect of Intra-articular Comminuted Fracture of AO-C Type Fracture of the Distal Humerus

[Abstract] Objective To study the clinical curative effect of Intra-articular comminuted fracture of AO-C type fracture of the distal humerus. Methods 56 cases of patients with intra-articular comminuted fracture of AO-C type fracture of the distal humerus admitted and treated in our hospital from April 2016 to April 2017 were convenient selected， 30 cases were treated with tension band fixation， while 26 cases were treated with Y shaped plate fixation treatment， and the treatment effect and occurrence of adverse reactions were counted. Results The total treatment effective rate in the double tension band was higher than that in the Y shaped plate fixation group （96.67% vs 80.77%）（χ2=8.976，P<0.05） ， and the treatment satisfactory rate was higher than that in the Y shaped plate fixation group （100.00% vs 92.31%）（χ2=9.042，P<0.05） ， and the incidence rate of adverse reactions was higher than that in the Y shaped plate fixation group （23.08% vs 10.00%）（χ2=7.485，P<0.05）. Conclusion The double tension band for internal fixation in treatment of intra-articular comminuted fracture of AO-C type fracture of the distal humerus has advantages， with simple operation and high safety， which is suitable for patients of various types， but the Y shaped plate is suitable for the patients with C1 fractures.

肱骨远端关节内粉碎性骨折是骨科诊室中多发疾病，因为闭合复位困难，应用开放复位内固定效果不理想，进而诱发肘关节功能出现障碍、骨不连甚至畸形愈合的情况[1]。对此方法主要治疗方法就是手术治疗，但是手术治疗难度较大，术后并发症较多，对于内固定治疗存在一定争议。该文主要探讨肱骨远端AO-C型骨折关节内粉碎性骨折的临床疗效，方便选择该院2016年4月—2017年4月收治的56例肱骨远端AO-C型骨折关节内粉碎性骨折患者，观察临床治疗效果，现报道如下。

1 资料与方法

1.1 一般资料

方便选择该院收治的56例肱骨远端AO-C型骨折关节内粉碎性骨折患者。男性患者：30例，女性患者26例，年龄19～73岁，平均年龄（60.43±1.32）岁。受伤原因：摔伤20例，车祸12例，打击伤13例，坠落伤11例。患者均为新鲜骨折，根据AO/ASIF对患者骨折类型分类均为C型，C1为20例，C2为26例，C3为10例。根据患者治疗方式的不同分成两组，双张力带组，患者30例，年龄（57.22±2.19）岁；男性16例，女性14例；C1为11例，C2为13例，C3为6例。Y形钢板组，患者26例，年龄（58.09±2.14）岁；……
