【摘要】 目的:探析在尿毒症患者中高通量血液透析的疗效和对C反应蛋白(CRP)、白细胞介素-6(IL-6)、β2-微球蛋白(β2-MG)水平的影响。方法:根据透析方式不同将2017年4月-2018年10月于笔者所在医院接受治疗的86例尿毒症患者,分为实施低通量血液透析的对比组43例和实施高通量透析的研究组43例,对比分析两组治疗6个月后的肾功能、炎症指标与疗效。结果:研究组治疗后的BUN、Scr含量水平与对比组比较,差异无统计学意义(P>0.05);研究组治疗后β2-MG为(17.19±4.18)mg/L,低于对比组,差异有统计学意义(P<0.05);研究组CRP(6.23±1.24)mg/L、IL-6(30.28±5.17)ng/ml,均低于对比组,差异均有统计学意义(P<0.05);研究组的总有效率为97.67%,高于对比组的74.42%,差异有统计学意义(P<0.05)。结论:在尿毒症患者透析治疗中使用高通量血液透析可促进患者肾功能的恢复,缓解患者炎症反应程度,提高治疗有效率。

Therapeutic Effect of High Flux Hemodialysis on Uremia Patients and Its Influence on Levels of CRP, IL-6 and β2-MG/GUO Chunxiang, YIN Jinlong. //Chinese and Foreign Medical Research, 2019, 17(28): -49
[Abstract] Objective: To explore the effect of high flux hemodialysis and the influence on the levels of C-reactive protein (CRP), interleukin-6 (IL-6) and beta-2-microglobulin (β2-MG) in uremic patients. Method: According to different dialysis methods, 86 uremic patients treated in our hospital were selected from April 2017 to October 2018. They were divided into the control group (43 cases, low flux hemodialysis) and the research group (43 cases, high flux hemodialysis). The renal function, inflammation index and curative effect of the two groups after 6 months of treatment were compared and analyzed. Result: There were no significant differences in BUN and Scr levels between the study group and the control group after treatment (P>0.05). After treatment, the level of β2-MG was (17.19±4.18) mg/L in the study group, lower than that in the control group, the difference was statistically significant (P<0.05). The levels of CRP was (6.23±1.24) mg/L and IL-6 was (30.28±5.17) ng/ml in the study group, lower than those in the control group, the differences were statistically significant (P<0.05). The total effective rate of the study group was 97.67%, higher than 74.42% of the control group, the difference was statistically significant (P<0.05). Conclusion: High flux hemodialysis can promote the recovery of renal function in uremic patients during dialysis treatment, alleviate the degree of inflammation in patients, and improve the efficiency of treatment.

尿毒癥是由于人体肾脏代谢功能衰竭所引发的代谢紊乱状况,使得机体无法通过肾脏将代谢废物及水分以尿液的形式排出而导致的疾病,该疾病临床主要表现为下肢水肿、肌肉痉挛、尿量减少等,使患者的生活质量受到严重影响[1-2]。临床中常以低通量血液透析等方式治疗尿毒症,虽可发挥清除作用但不能将大分子、中分子等物质彻底清除,使得患者进行透析后因部分毒性物质滞留于血清中而引发较多的并发症[3]。随着医疗技术的发展,高通量血液透析技术的出现可将尿毒症患者大分子代谢废物进行清除,为探析高通量血液透析对尿毒症患者的治疗效果,本文在尿毒症患者治疗中以高通量血液透析进行治疗并将内容报道如下。
1 资料与方法
1.1 一般资料
