类风湿关节炎患者血管内皮生长因子的变化及其相关性分析

【摘 要】目的：观察类风湿关节炎（RA）患者血管内皮生长因子（VEGF）的变化，并探讨VEGF与其他指标和中医症状的相关性。方法：选取38例活动期RA患者（活动组）、22例缓解期RA患者（缓解组）和20例正常体检者（正常对照组），观察3组VEGF变化，并探讨RA患者VEGF数值与血小板计数（PLT）、类风湿因子（RF）、红细胞沉降率（ESR）、C反应蛋白（CRP）等指标和中医症状之间的关系。结果：活动组VEGF水平显著高于缓解组和正常对照组，缓解组VEGF水平显著高于正常对照组，差异有统计学意义（P < 0.01）。活动组VEGF与PLT、ESR、CRP、RF呈正相关（P < 0.05），与IgM呈负相关（P < 0.05）;缓解组VEGF与PLT、ESR、CRP呈正相关（P < 0.05）;活动组VEGF与关节肿胀、关节疼痛、肢体困倦、舌暗苔膩呈正相关（P < 0.05）;缓解组VEGF与关节肿胀、关节疼痛呈正相关（P < 0.05）。结论：RA患者的VEGF与所选指标及中医症状有相关性，表明VEGF可在一定程度上反映RA疾病活动度，对临床判断疾病情况及用药疗效有一定价值。

Changes and Correlation of Vascular Endothelial Growth Factor in Patients with Rheumatoid Arthritis

【ABSTRACT】Objective：To observe the changes of vascular endothelial growth factor（VEGF）in patients with rheumatoid arthritis（RA），and to explore its correlation with other indicators and TCM symptoms.Methods：Thirty-eight active RA patients（the active group），22 remission RA patients（the remission group）and 20 normal subjects（the normal control group）were selected to observe the changes of VEGF，and to explore the relationship between VEGF value，platelet count（PLT），rheumatoid factor（RF），erythrocyte sedimentation rate（ESR），C reactive protein（CRP）and TCM symptoms in RA patients.Results：The VEGF level of RA patients in the active group was significantly higher than that in the remission group and the normal control group，and the VEGF level in the remission group was significantly higher than that in the normal control group，the difference being statistically significant（P < 0.01）.VEGF in the active group was positively correlated with PLT，ESR，CRP and RF（P < 0.05），and negatively correlated with IgM（P < 0.05）.

VEGF in the remission group was positively correlated with PLT，ESR and CRP（P < 0.05）.

VEGF in the active group was positively correlated with joint swelling，joint pain，cumbersome fatigued limbs and dark and greasy tongue（P < 0.05）.VEGF in the remission group was positively correlated with joint swelling and joint pain（P < 0.05）.Conclusion：VEGF in RA patients is correlated with the selected indicators and TCM symptoms，which indicates that VEGF can reflect the disease activity of RA to a certain extent，and has a certain value in clinical judgment of disease situation and drug efficacy.

类风湿关节炎（rheumatoid arthritis，RA）是有一定致残力的慢性疾病，关节内、外分别以滑膜炎、血管炎为病理改变，滑膜异常增生形成的血管翳是造成关节、骨质受损的重要因素[1]。血管内皮生长因子（VEGF）刺激血管生长，推动滑膜炎症反应和血管新生过程，加速了RA病程的进展[2]。近年来，VEGF对RA的影响成为研究热点，越来越多的研究表明，VEGF与RA发病联系紧密[3-5]。本研究通过观察RA患者的VEGF表达水平及变化，并选取代表性的实验室指标和中医症状进行相关性分析，进一步了解VEGF对RA的病情影响及评价作用。

1 临床资料

1.1 一般资料 选取2019年4月至2020年4月在安徽中医药大学第一附属医院风湿科住院的RA患者60例，根据DAS28评分[6]（≥2.6分为活动期，< 2.6分为缓解期）分为活动组38例和缓解组22例。活动组男15例，女23例;年龄21～74岁，平均（54.16±11.68）岁;病程1～42年，平均（18.53±12.91）年。缓解组男8例，女14例;年龄20～73岁，平均（48.95±14.84）岁;病程1.5～39年，平均（20.10±10.49）年。另设20例本院健康体检者为正常对照组，男7例，女13例;年龄20～70岁，平均（49.50±13.35）岁。

1.2 诊断标准 西医诊断按照2010年美国风湿病学会（ACR）和欧洲抗风湿病联盟（EULAR）提出的RA诊断标准。中医证型标准按照《中药新药临床研究指导原则》[7]，以脾虚湿盛、脉络痹阻型为基础证型，结合临床，症选：关节肿胀、关节疼痛、肢体困倦、纳食减少、口唇紫暗、舌暗苔腻。……
