经后方肱三头肌两侧肌间隙入路切开复位交叉克氏针固定治疗GartlandⅢ型儿童肱骨髁上骨折的临床疗效研究

【摘要】目的：探讨经后方肱三头肌两侧肌间隙入路切开复位交叉克氏针固定治疗GartlandⅢ型儿童肱骨髁上骨折的临床疗效。方法：回顾分析2017年1月至2019年6月共31 例经后方肱三头肌两侧肌间隙入路切开复位交叉克氏针固定治疗的GartlandⅢ型儿童肱骨髁上骨折的病历资料。结果：术后随访3 个月，依据Flynn肘关节功能评分标准评估治疗效果：功能恢复优26 例，良5 例，无恢复一般及差的病例，总体优良率100 %。结论：经后方肱三头肌两侧肌间隙入路切开复位交叉克氏针固定治疗GartlandⅢ型儿童肱骨髁上骨折療效显著。

The Clinical Efficacy study of open reduction and cross Kirschner wire fixation via posterior triceps bilateral intermuscular approach for Gartland typeⅢ children humerus condyle fracture

[Abstract] Objective： To explore the clinical efficacy study of open reduction and cross Kirschner wire fixation via posterior triceps bilateral intermuscular approach for Gartland type Ⅲ children humerus condyle fracture. Methods： retrospective analysis about 31 cases Medical records between January 2017 and June 2019， which accepted open reduction and cross Kirschner wire fixation via posterior triceps bilateral intermuscular approach. Results： After 3 months of postoperative follow-up， the therapeutic effect was evaluated according to Flynn elbow function score criteria： 26 cases with excellent functional recovery， 5 cases with good functional recovery， and no cases with general or poor functional recovery， with an overall excellent and good rate of 100%. Conclusion： The effect that open reduction and cross Kirschner wire fixation via posterior triceps bilateral intermuscular approach for Gartland type Ⅲ children humerus condyle fracture is remarkable.

肱骨髁上骨折是儿童最常见的骨折，手法复位石膏或经皮克氏针固定一般用于无明显移位或轻度移位的患者。对于移位明显的病例，手法复位有时难以成功，且有加重血管、神经损伤的风险，为避免肘关节功能恢复不良，越来越多的学者提出直接采用切开复位克氏针内固定治疗。笔者采用经后方肱三头肌两侧肌间隙入路切开复位内外侧交叉克氏针固定治疗GartlandⅢ型儿童肱骨髁上骨折，在解剖复位后牢固固定骨折断端，提高了解剖复位成功率，同时避免了医源性肱动脉、尺神经损伤风险，远期均未出现肘内外翻、骨化性肌炎等并发症，取得了良好效果。现对本科室2015年1月至2019年6月共31例采用该方法进行手术治疗的GartlandⅢ儿童肱骨髁上骨折进行回顾分析，总结如下。……
